女性HPV感染现状与年龄的分层分布研究 被引量:11

摘要 目的:研究女性人乳头瘤病毒(HPV)的感染现状及其与年龄的相关性。方法:收集大连地区4281例成年女性宫颈脱落细胞标本进行HPV定性检测并加以分型,结合年龄信息进行统计分析。结果:4 281例标本中,HPV阳性标本974例,阳性率为22.75%;在不同年龄分组中,以21-25岁感染率(40.67%)最高。共检出20种基因型,包括低危型HPV6、11、42、43和高危型HPV16、18、31、33、35、45、51、52、53、56、58、59、66、68、73、83,低危型以HPV43感染为主,高危型以HPV16感染为主。974例阳性标本中,单一感染689例(占70.74%),双重感染211例(占21.66%),多重感染77例(占7.91%);低危型感染166例(占17.04%),高危型感染692例(占71.05%),混合感染122例(占12.53%)。结论:大连地区女性HPV感染具有明显的亚型和年龄分布异质性,提示宫颈癌的筛查及防治应同时考虑HPV分型和年龄因素。 Objective: To research the current situation of human papillomavirus( HPV) infection in female patients and its correlation with age. Methods: The cervical exfoliated cell specimens of 4 281 adult women were collected in Dalian area,then HPV qualitative detection and typing were performed,a statistical analysis was conducted combing with age. Results: Among 4 281 specimens,974 specimens were found with positive HPV,the positive rate was 22. 75%; among different age groups,the infection rate in 21- 25- year- old group was the highest( 40. 67%); a total of 20 kinds of genotypes were detected,including low risk HPV 6,11,42,43 and high risk HPV16,18,31,33,35,45,51,52,53,56,58,59,66,68,73,83; the predominant low risk HPV genotype and high risk HPV genotype were HPV 43 and HPV 16,respectively; among 974 specimens with positive HPV,the proportions of single infection,double infection and multiple infection were 70. 74%( 689 specimens),21. 66%( 211 specimens) and 7. 91%( 77 specimens),respectively; the proportions of low risk infection,high risk infection and mixed infection were 17. 04%( 166 specimens),71. 05%( 692 specimens) and 12. 53%( 122specimens),respectively. Conclusion: The subtype- specific and age- specific distributions of HPV infection in Dalian area have obvious heterogeneity,which indicates that HPV genotypes and age should be considered in screening,prevention and treatment of cervical cancer.
