摘要
目的：探讨低分子肝素钙对儿童过敏性紫癜早期肾损害的临床疗效和用药安全性。方法：60例过敏性紫癜患儿分成对照组（30例）和观察组（30例）,对照组患儿均于口服氯雷他定（开瑞坦）、静脉滴注葡萄糖酸钙,伴随肠道症状患者静脉滴注氢化可的松琥珀酸钠;观察组在对照组治疗基础上皮下注射低分子肝素钙,1次/d,50~70 U/（kg·d）,以2周为1个疗程,1个疗程结束后对比两组患儿尿微量白蛋白（Alb）、尿β2-微球蛋白（β2-MG）、尿免疫球蛋白G（IgG）。结果：疗程结束后观察组患儿的β2-MG、尿IgG与对照组比较,差异具有统计学意义（P〈0.05）,观察组低于对照组。结论：低分子肝素钙对儿童过敏性紫癜早期肾损害有理想的预防作用,临床上建议进一步推广运用。
Objective To explore the curative effect and drug safety of low molecular heparin calcium on early renal impairment of Henoch-Sehonlein purpura（HSP） in children. Method 60 children with HSP were enrolled in the study and divided randomly into two groups. The control group（30 eases） received treatment：oral Loratadine（Clarityne） ,calcium gluconate by intravenous drip, hydroeortisone sodium suceinate by intravenous drip in patients with intestinal symptoms. In addition to the treatment of the control group,the observation group （30 eases） received low molecular heparin calcium by subcutaneous injection in the dose of 50 - 70 U/（ kg d） , once per day and 2 weeks were a course of treatment. After a course of treatment,the urine levels of mieroalbumin（mAlb） , β2 - microglobulin（ β2 - MG） and immunoglobulin G（IgG） were detected and compared in these two groups. Results There were statistically differences of the urine levels of mAlb, 132 - MG and IgG between two groups （ P 〈 0.05 ） , and the levels in the observation group were lower than those in the control group. Conclusion Low molecular heparin calcium has ideal effect on preventing carly renal impairment of HSP in children and we suggest it should be further popularization and application clinically.
