摘要
目的评价彩色多普勒(CDFI)、三维血管容积显像和超声造影成像在诊断移植肾慢性排斥(CR)的应用价值。方法选取移植肾患者119例,同时进行CDFI、三维血管容积显像和超声造影检查,ROC曲线分析各项参数评价CR的敏感度和特异度。结果 CR组RI、PI增高(P<0.05)、三维血管容积参数VI、FI、VFI均减低、MG增高(P<0.05)、超声造影参数皮、髓质的RT、TTP、mTT均延长(P<0.05);CR组ROC曲线下面积比较:RTc曲线下面积大于VI、FI、VFI、RI(P<0.05);当RTc=7.87s、VI=36.77﹪、FI=46.51﹪、VFI=23.18﹪、RI=0.62时诊断移植肾CR的敏感度分别为:90.2﹪、78.4﹪、75.7﹪、83.8﹪、65.6﹪,特异度分别为:75.6﹪、63.3﹪、73.3﹪、72.4﹪、73.5﹪。结论超声造影与三维血管容积显像诊断移植肾CR的敏感度及特异度高于CDFI;超声造影参数RTc评价移植肾CR的敏感度和特异度高于VI、FI、VFI、RI。
Objective The aim of this study was to evaluate the clinical value of color Doppler, three- dimensional imaging of vascular volume and contrast-enhanced ultrasound in chronic rejection. Methods One hundred and nineteen renal allograft patients underwent color doppler, three-dimensional imaging of vascular volume and contrast-enhanced ultrasound. The ROC curve was applied to acquire the sensitivity and specificity degrees. Results In the CR group, RI and PI of color Doppler increased (P〈0.05); VI, FI and VFI were decreased; MG was increased(P〈0.05); RT, TTP and mTT were extended(P〈0.05) among the parameters of contrast-enhanced ultrasound in the cortex and medulla(P〈0.05). The areas under the curve of RT in the cortex were greater than VI, FI, VFI, and RI(P〈0.05). In the most significant parameters, the diagnostic point of RT, VI, FI, VFI and RI were7.87%, 36.77%, 46.51%, 23.18% and 0.62% withsenstivityofg0.2%, 78.4%, 75.7%, 83.8% and 65.6% and with specificity of 75.6 % , 63.3%, 73.3%, 72.4% and 73.5%. Conclusion The sensitivity and specificity of contrast-enhanced ultrasound and three-dimensional imaging of vascular volume are better than color Doppler, and contrast-enhanced ultrasound is better than three-dimensional imaging of vascular volume in the diagnosis of chronic rejection of transplanted kidney.
