摘要
目的探讨青年缺血性脑卒中患者的病因以及影响其发生的危险因素。方法采用回顾性分析方法,选取2012年—2013年2月间于我院神经内科就诊并治疗的青年缺血性脑卒中患者76例,进行TOAST病因分型,同期选取行健康体检者80例作为对照组。对比分析2组临床资料,探讨影响青年缺血性脑卒中的危险因素。结果 76例青年缺血性脑卒中患者中,LAA 32例(42.11%),CE 5例(6.59%),SVO 26例(34.21%),SOE 4例(5.26%),SUE 9例(11.84%);青年缺血性脑卒中的危险因素包括高血压、糖尿病、高血脂、吸烟、酗酒、高同型半胱氨酸血症,在青年缺血性脑卒中患者中伴发上述症状者较对照组显著增加,组间差异具有统计学意义(P<0.05);对于伴发心脏病或具有脑卒中家族史而言,青年缺血性脑卒中组的发病率也较对照组升高,但差异无统计学意义(P>0.05)。结论大动脉粥样硬化及小血管闭塞是造成青年缺血性脑卒中的主要病因,此外,导致青年缺血性脑卒中的主要危险因素包括高血压、糖尿病、高血脂、吸烟、酗酒、高同型半胱氨酸血症。
Objective To investigate the young patients with ischemic stroke etiology and risk factors for its occurrence affected.Methods A retrospective analysis, selected in February 2012-in patients with ischemie stroke in February 2013 between the youth in our hospital neurology clinic and treatment of 76 cases,the cause be TOAST classification, compared with healthy subjects of the selected row 80 cases of individuals as a control group, comparative analysis of two clinical data, explore the risk factors of young ischemic stroke.Results 76 cases in young patients with ischemic stroke,LAA 32 例 (42.11%),CE 5例 (6.59%),SVO 26例 (34.21%),SOE 4 例 (5.26%),SUE 9cases (11.84% );risk factors for ischemic stroke in young people, including high blood pressure, diabetes, high cholesterol, smoking,alcoholism,high homocysteine in patients with ischemic stroke in young people associated with the above symptoms than the control was significantly increased, the difference was statistically significant(P〈0.05 )between the two groups; For concomitant heart disease or have a family history of stroke, the incidence of ischemic stroke in young group than in the control group also increased, but the difference was not statistically significant (P〉0.05).Conclusion large artery atherosclerosis and small vessel occlusion is caused by young major cause of ischemic stroke, in addition, a major risk factor for ischemic stroke in young people,including high blood pressure,diabetes,high cholesterol, smoking, alcoholism, high isotype hyperhomocysteinemia.
