摘要
目的:观察CT引导不同肺穿刺活检的临床效果。方法收集2007年5月至2013年1月行CT引导下经皮穿刺肺活检101例患者的临床资料,其中男63例,女38例,年龄15~72岁,平均57岁。所有患者均行CT引导肺穿刺活检,统计分析57例采用自动活检枪含抽吸针(FNA)与44例采用自动切割针插入带槽针芯(FNA+Core)方法的诊断结果与并发症的发生率。结果FNA术与FNA+Core术恶性疾病诊断率分别为77.2%、65.9%,准确率分别为93.2%、95.5%;良性疾病诊断率分别为22.8%、34.1%,准确率分别为84.6%、86.7%。FNA+Core术辅助诊断良性肺部病变的准确率明显高于FNA术(P<0.05),两种方法的并发症发生率比较,差异无统计学意义(χ2=0.68,P=0.912)。结论经皮肺穿刺活检技术可通过FNA术与FNA+Core术获取标本;且后者辅助诊断良性肺部病变的准确率明显高于前者。
Objective To explore clinical effects of different methods of CT-guided puncturations for lung bi-opsies .Methods A total of 101 patients ,including 63 males and 38 females ,with age from 15 to 72 years old(average 57 years old) ,receiving CT-guided lung biopsies from May 2007 to January 2013 were enrolled .Among all patients , 57 cases were treated by fine needle aspiration (FNA) and 44 were treated with FNA+Core .Diagnosis and incidence of complications were compared between the two groups .Results The diagnostic rates of malignant in FNA group and FNA+Core group were 77 .2% and 65 .9% ,and the accurate diagnostic rates were 93 .2% and 95 .5% .The diag-nostic rates of benign lesions in FNA group and FNA +Core group were 22 .8% and 34 .1% ,and the accurate diag-nostic rates were 84 .6% and 86 .7% ,respectively .The accurate diagnostic rate of benign lesions in FNA + Core group was higher than FNA group(P〈0 .05) ,but the difference of the incidence of complications between the two groups was not significant (χ2 =0 .68 ,P=0 .912) .Conclusion CT-guided lung biopsies might be able to be conduc-ted by FNA and FNA+Core technique ,and the latter could be more accurate for the diagnosis of benign lesions .
