血清胱抑素C与β_2微球蛋白在糖尿病肾病早期诊断中的意义 被引量:18

摘要 目的:探讨血清胱抑素C(CysC)与β2微球蛋白(β2-MG)在糖尿病肾病早期诊断中的临床价值。方法:对我院收治的134例糖尿病患者临床资料进行回顾性分析,根据是否合并肾病将其分为糖尿病无肾病组(DM组,68例)和糖尿病早期肾病组(DN组,76例),同时设立健康对照组,对3组尿微量白蛋白排泄率(UAER)及血清CysC、肌酐(Scr)、尿素氮(BUN)及β2-MG水平进行检测及比较。结果:DM组和DN组UAER及血清CysC、β2-MG明显高于健康对照组(P<0.05),且DN组UAER及血清CysC、β2-MG高于DM组(P<0.05),各组血清Scr和BUN差异均无统计学意义(P>0.05)。结论:早期糖尿病肾病血清CysC与β2-MG升高明显,且比血清Scr、BUN上升出现的时间早,可作为糖尿病肾病早期的预测指标。 Objective: To discuss the clinical value of serum cystatin C (CysC) and β2 microglobulin in in early diagnosis of diabetic patients with early nephropathy . Methods: Retrospective analysis of diabetic patients was carried out according to the complication with kidney. The patients were divided into two groups: diabetes without nephropathy group (DM group), early diabetic nephropathy group (DN group). At the same time, healthy control group was selected. UAER and serum CysC,serum creatinine (Scr), blood urea nitrogen (BUN) and β2-MG were detected. Results: UAER and serum CysC and β2-MG levels in DM group and DN group were significantly higher than those ot healthy control (P〈.0.05), and UAER and CysC and β2-MG levels in DN group were higher than those in DM group (P〈0.05). The differences in serum levels of Scr and BUN among three groups were not statistically significant (P〉 0.05). Oonclusions:The serum CysC and β2--MG in elderly patients with early diabetic nephropathy are increases significantly, and they could be used as predictors of early diabetic nephropathy.
