摘要
目的分析引起阴式手术后发热的相关因素,旨在探讨术后发热与感染等因素的相关性。方法回顾性分析2010年10月-2012年6月妇科行阴式手术的200例患者临床资料,采用logistic回归模型分析术后发热的影响因素。结果 200例阴式手术患者发生医院感染21例,感染率为10.5%;感染患者送检标本中共分离出病原菌16株,前3位病原菌依次为粪肠球菌、无乳链球菌、大肠埃希菌,分别占31.25%、25.00%、25.00%;术前尿路感染、阴道感染、手术出血量、手术时间等因素均是阴式手术患者发生医院感染的危险因素(P<0.05)。结论术前合并尿路感染及阴道感染,手术时间长且出血量多的患者术后更容易出现发热,通过控制相关因素,可有效预防阴式手术后感染性发热。
OBJECTIVE To analyze the related factors for fever after vaginal surgery and observe the relationship between the postoperative fever and the infections.METHODS The clinical data of 200patients who underwent the vaginal surgery in the department of gynecology from Oct 2010to Jun 2012were retrospectively analyzed,then the influencing factors for the postoperative fever were analyzed by means of the logistic regression model.RESULTS The nosocomial infections occurred in 21of the 200patients who underwent the vaginal surgery with the infection rate of 10.5%.A total of 16strains of pathogens have been isolated from the 21cases of nosocomial infections, among which the Enterococcus faecalis,Streptococcus agalactiae,and Escherichia coli ranked the top three species,accounting for 31.25%,25.00%,and 25.00%,respectively.The preoperative urinary tract infections,vaginal infections,volume of blood loss,and operation duration were the risk factors for nosocomial infections in the patients undergoing vaginal surgery（P＆lt;0.05%）.CONCLUSIONThe postoperative fever is prone to occur in the patients complicated with preoperative urinary tract infection,vaginal infections,long operation duration,or large volume of blood loss;the postoperative infectious fever can be effectively prevented through the control of related factors.
