锁定解剖钢板内固定治疗陈旧性桡骨远端骨折 被引量:8

Internal fixation with anatomical locked plates for the delayed distal radius fractures

摘要 目的 探讨锁定解剖钢板内固定治疗陈旧性桡骨远端骨折的疗效.方法 采用锁定解剖钢板内固定治疗25例陈旧性桡骨远端骨折患者;测量手术前后腕关节活动度、握力、桡骨短缩、掌倾角、尺偏角、采用Fernandez评分评价疗效.结果 25例均获随访,时间16~24周.患者术后的影像学检测和功能评分较术前均有显著改善(P<0.05).掌倾角由术前-15°±5°纠正为术后13°±4°,尺偏角由术前11°±4°纠正为术后22°±3°;腕关节屈曲活动度从术前42°±12°恢复到术后51°±14°;桡骨短缩长度由术前12 mm±3.8 mm纠正为术后2 mm±1.2 mm;握力从术前健侧的35%±4.3%恢复到术后健侧的80%±5.2%.功能评分由术前12分±3.8分增加到术后18分±4.9分.结论 锁定解剖钢板内固定治疗陈旧性桡骨远端骨折是一种有效的固定方法. Objective To evaluate the effects of the delayed distal radius fractures fixed with anatomical locked plates. Methods 25 cases of the delayed distal radius fractures fixed with anatomical locked plates were reviewed, the radiological measurement, the motion of the wrist joint, the grip strength, the Fernandez scores were evaluated. Results All cases were followed up for 16 ~ 24 weeks. There was a significant improvement in the anatomical and functional parameters (P 〈0. 05). The mean tilt of the radius improved from - 15° ± 5° to 13° ± 4° , the mean ra dial inclination improved from 11° ± 4° to 22° ± 3°. Wrist flexion range of motion improved from 42° ± 12° to 51° ± 14°. The mean shorten lengthen improved from 12mm ± 3.8mm to 2 mm ± 1.2 mm. Grip strength increased from 35% ± 4. 3% to 80% ± 5.2%, the Fernandez scores improved from 12 ± 3.8 to 18 ± 4. 9. Conclusions Fixation with anatomical locked plates for the delayed distal fracture of radius might be an effective method.
