摘要
目的研究及探讨交锁髓内钉早期动力化在促进胫骨骨折愈合及控制不良情况发生中的效果,以为其应用提供依据。方法选取2011年1月—2012年12月于浙江大学医学院附属第二医院进行治疗的78例胫骨骨折患者为研究对象,将78例以随机分配的方式分为对照组(常规交锁髓内钉治疗组)39例和观察组(交锁髓内钉早期动力化治疗组)39例,然后将2组中不同AO/ASIF分型者的Rasmussen评分结果与不良情况发生率、治疗前后的血清VEGF、EGF、足甲襞微循环指标进行比较。结果观察组中不同AO/ASIF分型者的Rasmussen评分结果优于对照组,不良情况发生率低于对照组,血清VEGF、EGF、足甲襞微循环指标检测水平也明显好于对照组,P均<0.05,2组患者治疗后的比较项目之间均有显著性差异。结论交锁髓内钉早期动力化不仅可有效促进胫骨骨折愈合,而且对控制不良情况发生也发挥着积极的临床作用,因此其临床综合应用价值较高。
Objective To study the effect of early dynamic of interlocking intramedullary nail in promoting the healing and controlling the bad situation of patients with tibial fracture, in order to provide evidence for the application. Methods 78 patients with tibial fracture in our hospital from January 2011 to December 2012 were selected as research object, and 78 cases were randomly divided into control group （ routine interlocking intramedullary nail treatment group） and observation group（ early dynamic of interlocking intramedullary nail group）, with 39 cases in each group, then the Rasmussen score of different AO/ASIF types and bad situation rate, serum VEGF, EGF,foot nailfold microcirculation be- fore and after the treatment of two groups were compared. Results The Rasmussen score of observation group with differ- ent AO/ASIF types were all better than those of control group, bad situation rates were lower than those of control group, serum VEGF, EGF, foot nailfold microcirculation were obviously better than those of control group too, with all P 〈 0. 05, there were all significant differences between those indexes of two groups after the treatment. Conclusion The early dy- namic of interlocking intramedullary nail can not only effectively promote the healing of patients with tibial fracture, but al- so played an active role in controlling the bad situation, so its clinical application value was high.
