摘要
目的既往研究表明,2型糖尿病患者心率变异降低明显,本研究采用24小时动态心电图探讨心率变异性与2型糖尿病的关系。方法选取42例经口服葡萄糖耐量检查(OGTT)确诊的2型糖尿病患者为观察组分析对象,另选经OGTT检查糖代谢正常的75例患者为对照组,对上述117例患者均进行24小时动态心电图检查,分析比较2组患者的心率变异指标,包括SDNN,RMSSD,pNN50,SDNN index和SDANN。结果对照组的SDNN为(121.61±34.49)ms,RMSSD为(54.07±31.91)ms,pNN50为(10.13±10.00)%,SDNN index为(41.92±20.47)ms,2型糖尿病组SDNN为(103.93±29.98)ms,RMSSD为(44.22±19.62)ms,pNN50为(5.85±5.69)%,SDNN index为(32.04±11.2)ms,2组之间的上述心率变异指标差异有统计学意义(P<0.05)。SDANN在对照组为(72.90±56.81)ms,在2型糖尿病组为(64.10±45.11)ms,2组之间差异无统计学意义(P>0.05)。结论 2型糖尿病患者心率变异性明显降低,可能与患者迷走神经功能受损有关。
Objective The past studies have demonstrated that heart rate variability （HRV） was significantly decreased in type 2 diabetes mellitus （T2DM） patients. The current study aimed to investigate the relationship between T2DM and HRV by using 24 hour dynamic echocardiogram. Methods 42 cases of T2DM confirmed by oral glucose tolerant test （OGTT） were enrolled in this study as observation group,while 75 subjects without abnormal glucose metabolism were re- cruited as control group. 24-hour Hoher electrocardiography was performed in all subjects. SDNN, RMSSD, pNNS0, SDNN index and SDANN were analyzed and compared. Results SDNN, RMSSD, pNN50 and SDNN index were （ 121.61 ± 34.49） ms, （54.07 _± 31.91 ） ms, （ 10.13 ± 10.00） % and （41.92 ± 20.47 ） ms, respectively, in the control group, and （ 103.93 ± 29.98 ） ms, （44.22 ± 19.62 ） ms, （ 5.85 ± 5.69 ） % and （ 32.04 ± 11.2 ） ms, respectively, in the observation group, the difference in the above HRV indexes were significant between the two groups （ P 〈 0. 05 ）. SDANN was （72.90 ± 56.81 ）ms in the control group and （64. l0 ± 45. 11 ）ms in the observation group, there was no significant difference between the two groups（P 〉 0.05 ）. Conclusion The reduced HRV indexes （except SDANN） in T2DM pa- tients may be possibly associated with the impaired vagus nerve.
