摘要
目的:探讨完全腹腔镜联合纤维胆道镜保胆取石术治疗胆囊结石的临床疗效和应用价值。方法:选择我院2011年1月至2012年6月间收治的胆囊结石行完全腹腔镜联合纤维胆道镜保胆取石术治疗的患者46例作为研究组,并选取同期腹腔镜胆囊切除术治疗的患者58例作为对照组。比较两组患者术中及术后情况、住院时间、复发率。结果:①两组患者性别、年龄和症状构成比较无统计学差异(P>0.05)。②两组均无中转开腹病例,两组患者术中、术后并发症比较无统计学差异(P>0.05);研究组手术时间明显长于对照组(P<0.05),研究组住院时间短于对照组(P<0.05)。③随访12-24个月,两组均无复发和死亡,研究组患者无严重不良反应,对照组患者出现进食后腹胀5例、腹泻1例、腹痛2例,不良反应发生率明显高于研究组(P<0.05)。结论:完全腹腔镜联合纤维胆道镜保胆取石术治疗胆囊结石安全有效,患者术中术后并发症较低,值得临床推广应用。
Objective：To evaluate clinical efficacy and value of laparoscopy combined with fiber chole-dochoscope for preservation of gallbladder surgery of cholecystolithiasis patients .Method： Selected 46 gall-bladder stones patients in our hospital from Jan .2011 to Jun.2012 that treated laparoscopy combined with fi-ber choledochoscope for preservation of gallbladder surgery as study group , and 58 patients of laparoscopic cholecystectomy as control group .Compared intraoperative and postoperative situation , hospital stay , recur-rence rate of two groups .Result： ① the gender , age of two groups of patients and symptoms constitute no statistical differences （ P＆gt;0.05） .The two groups were no case was converted to laparotomy , no significant differences between the two groups of patients , postoperative complications （ P＆gt;0.05）;the study group oper-ation time was significantly longer than that of the control group （P＆lt;0.05）, the study group hospitalization time was shorter than the control group （P＆lt;0.05）.The follow-up of 12-24 months, the two groups had no recurrence and death , no serious adverse reaction of patients in the study group , 2 cases in the control group patients with abdominal distention after eating in 5 cases, 1 case of diarrhea , abdominal pain , adverse reac-tion rate was significantly higher than that in study group （ P＆lt;0.05 ） .Conclusion： Laparoscopic combined with fiber choledochoscope for preservation of gallbladder stone surgery is safe and effective in patients with gallstone , intraoperative and postoperative complications is low , it is worthy of clinical application .
