摘要
目的:通过研究腹泻型肠易激综合征中医证候和血清脑肠肽的关系,进而以脑肠轴为切入点,探讨肝主疏泄影响脾胃运化的神经内分泌机制,并为腹泻型肠易激综合征中医辨证论治的客观化提供依据。方法:检测患者生长抑素(SS)、乙酰胆碱(Ach)、降钙素基因相关肽(CGRP)、血管活性肠肽(VIP)、5羟色胺3受体(5-HT3R)、多巴胺(DA)、P物质(SP)、内皮素-1(ET-1)等含量,运用SPSS 19.0进行证候与指标相关性的统计分析。结果:肝郁脾虚证与CGRP的相关性P<0.05;肝郁气滞证与Ach、VIP、5-HT3R的相关性P<0.05;脾胃虚弱证与CGRP的相关性P<0.05;脾胃湿热证与SP、ET-1的相关性P<0.01;寒湿困脾证与ET-1的相关性P<0.05;DA、SS与IBS-D各证候的相关性P>0.05。结论:Ach、CGRP、VIP、5-HT3R与IBS-D肝郁气滞证、肝郁脾虚证及脾胃虚弱证等密切相关,可能是体现肝脾两脏功能关系的重要物质基础。
Objective： Based on the correlation between brain-gut peptides and TCM syndromes of Irritable Bowel Syndrome （IBS）, to discuss the central mechanism that liver ruling smooth qi flow could assist the transportation and transformation of spleen and stomach. Method： Value of somatostatin（SS）, acetyl choline （Ach）, calcitonin gene-related peptide（CGRP）, vasoactive intestinal peptide（VIP）, 5-hydroxytryptamine receptor 3（5-HT3R）, dopamine（DA）, substance P（SP）, Endothelin-l（ET-1） were measured, then the correlation between the value of brain-gut peptides and TCM syndromes of IBS was analyzed by SPSS 19.0. Result： Stagnation of liver qi and deficiency in the spleen syndrome was related to CGRP P〈0.05; Stagnation of liver qi syndrome was related to Ach, VIP, 5-HT3R P〈0.05; Hypofunction of the spleen and stomach syndrome was related to CGRP P〈0.05; Dampness-heat in the spleen and stomach syndrome was related to SP, ET-1 P〈0.05; Cole-dampness fettering the spleen syndrome was related to ET-1 P〈0.05; DA and SS had no correlation to syndromes of IBS-D P〉0.05. Conclusion： Ach, CGRP, VIP, 5-HT3R and ET-1 are closely related to the TCM patterns of IBS-D such as stagnation of liver qi, stagnation of liver qi and deficiency in the spleen, and deficiency in the spleen and stomach. This might be the important material basis which could give expression to relationship between liver and spleen function.
