冠心病血瘀证Fisher函数诊断的临床研究

摘要 目的:结合临床指标进行冠心病血瘀证的计量诊断研究。方法:按冠心病血瘀证组68例、冠心病非血瘀证组64例及健康对照组64名,分别采集临床资料。运用逐步判别分析法对所有检测资料进行统计检验和判别分析,筛选判别指标,并建立各证型的Fisher判别函数;应用回代性检验和前瞻性检验以评价所建判别各证型Fisher函数的判别效果。结果:从血糖、血脂、血压、身体质量指数、心电图、冠状动脉造影等6类检测的13项指标中筛选出胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、身体质量指数、QT离散度、JT离散度、Gensini积分等7项,建立冠心病血瘀证、冠心病非血瘀证与正常对照组的Fisher判别函数式,回代性检验符合率为92.35%,前瞻性检验符合率为83.33%。结论:所建立的Fisher判别函数能较准确地做出冠心病血瘀证的诊断,所入选的指标对深入研究冠心病血瘀证形成的机制有启示意义。 Objective: To study the quantitative diagnosis of the blood-stasis syndrome in coronary heart disease (CHD) with the clinical index. Methods: A total of 196 cases were divided into three groups: blood-stasis syndrome of coronary heart disease group with 68 cases, non-blood-stasis syndrome of coronary heart disease group with 64 cases and normal control group with 64 cases. The clinical data was collected respectively. The statistical checks and judgement analysis were made to all of the detected information by stepwise method, the judge markers were selected, and the Fisher judgement function for the syndrome types was set up. Then the judge effect of the Fisher function for the syndrome was assessed by the backward check method and the forward check method. Results: From 13 kinds of 6 types experimental detects, such as blood glucose, blood lipid, blood pressure, body mass index (BMI), electrocardiogram, coronary angiography, 7 kinds of markers were selected: total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), body mass index, QT dispersion, JT dispersion, Gensini score. Then setting up the Fisher judgement function for the blood-stasis syndrome and the non-blood-stasis syndrome of CHD. As a result, the backward check coincidence was 92.35%, and the forward check coincidence was 83.33%. Conclusion: Correct diagnosis can be made for the blood-stasis syndrome of CHD by the Fisher judgement function relatively. It might be concluded from the above result that the selected markers are valuable for the deeper study on the mechanism of syndromes of CHD.
