摘要
目的：探讨透明晶状体超声乳化联合人工晶状体植入术治疗老年高度近视的安全性及临床疗效的观察。方法对60只眼已行老年高度近视白内障超声乳化术眼的对侧眼即透明晶状体眼进行透明角膜切口行超声乳化联合人工晶体植入术,患者年龄为（62-76）岁,平均（54±4.64）岁,平均等效球镜度数为（8.23±4.83）D,平均眼轴长度（29.44±2.06）mm,植入人工晶体度数为（6.23±3.83）D,随访12-24个月。结果术后60只眼最佳矫正视力均超过术前最佳矫正视力,术后裸眼视力达到或超过0.5者24只眼（40.00%）,52只眼（86.67%）予矫正屈光度±1.0 D以内,术后平均屈光度为（8.24±4.84）D,术中未发生并发症,术后12-24个月随访,其中,4只眼（6.67%）发生后发障,术后未发生角膜水肿、人工晶状体偏位、视网膜裂孔形成、视网膜脱离、黄斑出血等严重的并发症。结论透明晶状体超声乳化联合人工晶状体植入治疗老年高度近视与目前的角膜屈光手术相比,可能更适合戴镜不能矫治高度近视眼患者,具有安全、有效、并发症少、术后视力好等优点,其远期效果需长期随访和观察。
Objective To observe the clinical effect and safety of transparent lens phacoemulsification combined with intraocular lens implantation in the treatment of elderly patients with high myopia. Methods On 60 eyes with high myopia has been in senile cataract phacoemulsification contralateral eye eye eye lens that was transparent transparent corneal incision phacoemulsification and intraocular lens implantation, patients aged 62-76, the average （54±4.64）, the average spherical equivalent degree of （8.23±4.83）D, the average axial length （29.44±2.06）, implantation of intraocular lens power was （6.23±3.83）D, during the follow-up of 12-24 months. Results After operation, 60 eyes of best corrected visual acuity of more than preoperative best corrected visual acuity, postoperative uncorrected visual acuity reached or exceeded 0.5 in 24 eyes （40.00%）, in 52 eyes （86.67%） treated with internal corrective diopter ±1 D, the average refractive surgery is （8.24±4.84）D, No complications occurred during the operation, followed up for 12-24 months, after operation, 4 eyes （6.67%） after cataract, severe complications did not occur without postoperative corneal edema, intraocular lens shift, the formation of retinal hole, retinal detachment. Conclusion Transparent lens phacoemulsification with intraocular lens implantation therapy in elderly high myopia compared with current corneal refractive surgery, may be more suitable for patients with high myopia wear glasses can＆#39;t correction, is a safe, effective, less complications, postoperative visual acuity is good, its long-term effect needs long-term follow-up observation.
