摘要
目的 观察薤葶合剂治疗慢性阻塞性肺病发作期及对肺动脉高压的临床疗效并探讨其作用机制。方法 治疗组用薤葶合剂 ,对照组服用头孢拉啶、氨茶碱、必嗽平 ,2组对照观察。结果 治疗组总有效率及控显率分别为 92 .6 %、6 4.7% ,明显优于对照组 (P <0 .0 5 ) ;并能显著地改善肺功能 ,增加PaO2 、mPAP ,改善血液流变性 ,降低LPO、升高SOD。对常压缺氧的大鼠引起的肺动脉高压 ,有明显的降压作用 ,与心痛定和生理盐水对照组比较 ,有显著性意义 (P <0 .0 5 ) ,并可降低大鼠血浆TXB2 ,提高PGI2 含量 ,其作用明显优于对照组 (P <0 .0 5 )。结论 薤葶合剂治疗慢性阻塞性肺病发作期及对肺动脉高压疗效满意 ,其作用机理可能与促进排痰、解除支气管痉挛 ,改善通气和通气血流比例、调节TXA2 /PGI2 比值、改善血液流变性、调节自由基反应有关。
OBJECTIVE To observe the clinical effect of the treatment of chronic obstructive pulmonary disease during the acute phase with Xieting Mixture and to study the mechanism of the mixture. METHOD The patients in the treatment group took Xieting Mixture compared with a control group taking cefradine, aminophylline, and brombexine hydrochloride. RESULT The total effective rate and marked control rate achieved in the treatment group were 92.6% and 64.7% respectively, which markedly exceeded those in the control group ( P <0.05). The mixture can markedly improve pulmonary function, raise PaO 2 and mPAP, improve blood rheology, reduce LPO and increase SOD. It also has marked effect of reducing blood pressure in rats with pulmonary hypertension, with marked significance ( P <0.05) compared with a control group treated with nifedipine and normal saline. It reduces TXB 2 in rat's plasma and increases the content of PGI 2, and the effect is more satisfactory than that in the control group ( P <0.05). CONCLUSION Xieting Mixture has satisfactory effect in the treatment of chronic obstructive pulmonary disease during the acute phase and pulmonary hypertension. The mechanism may lie in its functions to promote the removal of sputum, relieve the spasm of the bronchus, promote the flow of qi, regulate the TXA 2/PGI 2 ratio, improve blood rheology, prevent the imbalance of free radical reaction, and reduce the pulmonary blood pressure.
