摘要
目的:探讨肿瘤坏死因子-α(tumornecrosisfactor-α,TN-α)基因308位点多态性与乳腺癌易感性的关系。方法:通过检索万方、维普、Medline、CNKI、PubMed和SpringerLink数据库,获取1990—01—2013-05关于TNF-α-308基因多态性与女性乳腺癌遗传易感性关系的研究结果,以乳腺癌组与对照组人群基因型分布的OR值为效应指标,采用STATAll进行Meta分析及检验,并根据一致性检验的结果选用合适的模型(固定效应模型或随机效应模型)进行数据的合并。使用漏斗图来进行发表偏倚的评估,使用Egger’s检验行发表偏倚的检验。结果:Meta分析共纳入16篇文献,累计24548例患者,包括10795例肿瘤患者和13753例对照,G等位基因相对于A等位基因合并0R值为1.12,95%cI为1.07~1.16,P〈0.001。亚组分析中欧美洲人G等位基因相对于A等位基因0R值为1.15,95%CI为1.10~1.20,P〈0.001,亚洲人G等位基因相对于A等位基因0R值为0.86,95%CI为0.66~1.11,P〈0.001,非洲人G等位基因相对于A等住基因0R值为0.64,95%CI为0.52~0.81,P〈0.001。结论:TNFα-308等位基因提高了乳腺癌易感性尤其是对欧美洲人,但对亚洲人来说没有明显关联性,对非洲人来说却能够降低乳腺癌的易感性。
OBJECTIVE: To study the relationship of genetic polymorphism of TNF-α -308 and breast cancer risk. METHODS: All the published studies about TNF-α-308 and breast cancer from 1990-01 to 2013-05 were reviewed and standard techniques of recta-analysis was used in the results of these studies to estimate the significance of the expression of TNF-α-308 more precisely. Meta-analysis odds ratios (ORs) were estimated using a fixed effects model or random effects model RESULTS: Sixteen eligible reports were identified, concerning 10 795 cancer cases and 13 753 controls. The pooled OR of G allele compared with that of A allele was 1.12 (95%CI: 1.07-1.16, P〈0. 001). The pooled OR of G allele compared with that of A allele for Caucasian was 1.15(95%CI: 1.10-1.20, P〈0. 001). The pooled OR of G allele compared with that of A allele for Asian was 0.86 (95%CI: 0. 66-1.11, P〈0. 001). The pooled OR of G allele compared with that of A allele for African was 0.64(95%CI: 0. 52-0. 81, P〈0. 001). CONCLUSIONS: This meta-anal- ysis suggests that the TNFα -308 G allele is a risk factor for developing breast cancer, especially for Caucasian. TNFα-308 G allele may not be significantly associated with Asian. However, G allele was associated with a decreased risk of breast cancer in African. Further studies are needed to explore gene-gene and gene-environment interactions.
