摘要
目的 探讨胆总管切开探查取石后不放置“T”形管的可行性及临床适应证。方法 在动物实验的基础上 ,选择 2 5 8例患者于胆总管切开探查取石后不放置“T”形管 ,并与传统的放置“T”形管法进行比较。结果 不放置“T”形管组的患者术后住院时间平均为 7 5d ,放“T”形管者为 16d ;不放置“T”形管的患者避免了“T”形管引起的诸多并发症。结论 在正确掌握适应证的前提下 ,胆总管切开探查后以不放置“T”形管为好。
Objective [WT5”BZ]To evaluate the rationale and indications for primary suture of the incision without the placement of a T tube after choledochotomy.[WT5”HZ]Methods [WT5”BZ] 258 patients undergoing choledochotomy for gallstone extraction received primary suture on the incision of the common bile duct without a T tube placement when it was identified that there were not residual stones.Result was compared with that of traditional procedure. [WT5”HZ]Results [WT5”BZ] Postoperative hospital stay was averagely 7 5 days, compared to 16 days in patients with a T tube drainage.Complications often seen in T tube drainage were avoided.[WT5”HZ]Conclusion [WT5”BZ] When properly applied, primary suture on the incision of CBD is safe, less traumatic, and indicative for patients undergoing negative choledochotomy or in which no stones, nor end stricture of the CBD could be identified. [WT5”HZ]
