摘要
目的 主要探讨老年哮喘气道反应性及治疗特点。 方法 老年哮喘 5 0例 (激素治疗组 2 8例及非激素治疗组 2 2例 )、慢性阻塞性肺疾病 (COPD) 48例、健康老年人 3 0例。观察肺活量 (VC)、一秒率 (FEV1/FVC % )、一秒量 (FEV1)及支气管扩张试验的疗效。 结果 老年哮喘者VC、FVE1、FEV1/FVC %与COPD病人相差不明显 (P >0 0 5 ) ,但低于健康老年人 (P <0 0 5 )。支气管扩张试验 ,老年哮喘高于COPD及健康老年人 (P <0 0 1)。激素组哮喘患者VC、FEV1、FEV1/FVC %与非激素组无明显差别 (P >0 0 5 )。FEV1变异率 ,激素组较非激素组病人改善明显 (P <0 0 5 )。 结论 支气管扩张试验是鉴别及诊断老年哮喘有价值的方法 。
Objective To explore the characteristics of airway reactivity and effects of steroid treatment on the elderly asthma. Methods We had observed VC,FEV 1,FEV 1/FVC%,MMEF and variability of FEV 1 after bronchial dilator inhalation in 50 asthmatic patients in the elderly(including 28 cases of steroid treatment and 22 cases of non steroid treatment),48 patients with chronic obstructive pulmonary disease(COPD) and 30 healthy elderly subjects.Results The VC,FEV 1,FEV 1/FVC% and MMEF in the elderly asthma were not signifficantly different from COPD( P >0.05),but lower than the healthy elderly subjects( P <0 05).After inhalation of salbutamol,the variability of FEV 1 in the elderly asthma group statistically higher than COPD( P <0 01)and the healthy elderly subjects( P <0 01).The variability of FEV 1 in the elderly asthma group with steroid treatment presented more significant improvement than those of patients without steroid treatment( P <0 05).Conclusions The test of bronchial dilation is a very useful method for diagnosis of the elderly asthma.Glucocorticoid can improve the reactivity of airway and relieve the symptoms.
