摘要
目的探讨免疫固定电泳免疫-削减法在多发性骨髓瘤患者辅助诊断中的应用。方法 60例骨髓涂片确诊为多发性骨髓瘤患者,分别对其血清进行免疫固定电泳和特种免疫球蛋白定量测定,并对两组结果进行比较。结果在60例MM患者中,免疫固定电泳检测结果显示IgG型占56.7%(34/60),IgA型占16.7%(10/60),IgM型占18.3%(11/60),单纯轻链型占5%(3/60)。特种免疫球蛋白定量结果显示,在各型MM患者血清中相应免疫球蛋白升高的比例分别为:IgG为21/34,IgA为8/10,IgM为11/11。免疫固定电泳对IgG型和IgA型MM患者的检出率明显高于特种免疫球蛋白定量分析,差异具有统计学意义(P<0.05)。结论在多发性骨髓瘤的实验室诊断中,免疫固定电泳对多发性骨髓瘤的检出率优于特种免疫球蛋白定量检测。
Objective To explore the application of M protein test by immunofixation electrophoresis immunosub- stractive method in patients with multiple myeloma. Methods Immunofixation electrophoresis and quantitative determina- tion of immunoglobulins were performed in 60 patients with multiple myeloma(MM), and the efficiency of the two meth- ods were compared. Results Immunophenotypes by immunofixation electrophoresis included IgG (56.7%, 34/60), IgA (16.7% ,10/60), IgM(18.3% ,11/60), and pure light chain type (5% ,3/60), respectively. The proportion of different im- munophenotypes with increased immunoglobulins were IGG(21/34), IgA (8/10), and IgM (11/11). The detection rate of IgG and IgA type MM by immune fixation electrophoresis was significantly higher than that by quantitative determination of immunoglobulins (P〈0. 05). Conclusion The detection of multiple myeloma by immune fixation electrophoresis is much better than that by quantitative determination of immunoglobulins.
