大斜面离断式肾盂成形术60例分析 被引量:1

摘要 目的探索大斜面离断式肾盂成形术治疗肾积水的可行性。方法自2006年10月-2011年10月以来,对60例肾盂输尿管连接部梗阻的患儿实行大斜面离断性肾盂成形术:显露肾盂输尿管连接部,切除梗阻的连接部及裁剪扩大的肾盂,将远端输尿管自内上向外下斜行裁剪,内侧比外侧高1cm左右,形成自内上向外下的大斜面,与肾盂吻合成形,自肾盂与膀胱之间放置双J管,经尿道留置导尿管。结果术后7d拔除导尿管,术后1个月拔除双J管;术后6-12个月做SPECT,B超及静脉尿路造影复查肾脏恢复的情况:影像学表现有不同程度的改善,无1例患儿出现再次梗阻现象。结论大斜面离断式肾盂成形术具有吻合口宽广,不易梗阻,手术成功率高的特点,值得在临床推广应用。
