交感电风暴危险因素的研究及艾司洛尔治疗的安全性观察 被引量:6

摘要 目的:评价超敏C反应蛋白、脑钠肽前体、肌钙蛋白等生化指标对交感电风暴的预测作用及评价艾司洛尔对交感电风暴的治疗安全性。方法:选取2010年3月-2013年5月间我院ICU收治的反复发作恶性心律失常29例,其中急性冠脉综合征21例,重症心肌炎7例,长QT综合征1例,对照组30例,为随机挑选心血管内科普通住院患者,住院期间无心律失常发作且均无其他系统严重疾病。两组进行回顾性分析。结果:交感电风暴组患者的超敏C反应蛋白、脑钠肽前体及肌钙蛋白均较对照组高,差异有统计学意义。29例患者应用艾司洛尔治疗,最终抢救成功27例,死亡2例。结论:超敏C反应蛋白、脑钠肽前体、肌钙蛋白对交感电风暴的发生有一定的预测作用。合理使用艾司洛尔能有效控制恶性心律失常的发作,降低死亡率,安全性高。 To evaluate the hs-CRP ,brain natriuretic peptide precursor ,troponin and other biochemical inde-xes for predicting the sympathetic electrical storm and the clinical safety of esmolol in the treatment of it .Methods :We picked 29 patients who were treated in our ICU from March 2010 to May 2013 .Including 21 acute coronary syndrome , 7 myocarditis ,1 long QT sydrome .It is a retrospective analysis .Results:hs-CRP ,brain natriuretic peptide and cardiac troponin in patients with sympathetic electrical storm is higher than the control group ,there was a significant differ-ence .Conclusion:hs-CRP ,brain natriuretic peptide precursor ,troponin is predictive to sympathetic electrical storm . The rational use of esmolol can effectively control the malignant ventricular arrhythmia ,reduce the mortality rate ,and it is high safety .
