摘要
目的探讨阿司匹林联合氯吡格雷对脑梗死急性期进展的影响。方法选取2008-02—2013-02急性脑梗死患者120例,按随机数字表法分为观察组和对照组各60例,2组均给予常规治疗,观察组加用阿司匹林和氯吡格雷,对照组加用阿司匹林。入院时及入院后第1、3、7天进行NIHSS评分及疗效评价。结果治疗后,观察组符合脑梗死病情进展标准11例(18.33%),对照组28例(46.67%),观察组病情进展率明显低于对照组(χ2=4.873,P=0.013)。治疗前2组NIHSS评分无显著性差异(P>0.05),治疗后2组均较治疗前明显改善,观察组改善情况明显优于对照组,差异有统计学意义(P<0.05)。观察组与对照组总有效率分别为56.67%、33.33%,观察组疗效明显优于对照组,差异有统计学意义(P<0.05)。2组均无严重不良反应发生。结论阿司匹林联合氯吡格雷治疗脑梗死急性期的临床疗效优于单用阿司匹林,可延缓病情的进展。
[Abstract] Objective To investigate the influence of aspirin combined with clopidogrel on the progress of acute phase of cerebral infarction. Methods 120 patients with acute cerebral infarction from February 2008 to February 2012 were randomly divided into observation group and control group , 60 cases in eachgroup. The two groups were given conventional treatment, the observation group was added with aspirin and clopidogrel, the control group used aspirin. NIHSS score and efficacy evalua- tion were proceeded on admission and 1, 3 and 7 days after admission. Results After treatment, patients in the observation group in accord with the standard of cerebral infarction progression was 11 cases （18.33 %） and that in control group was 28 cases （46. 67 %）, the progression rate of observation group was obviously lower than that of control group X2= 4. 873, P = 4. 873）. There was no significant difference between the NIHSS scores of the two groups （P〉0.05）;After treatment, twogroups were markedly improved, the observation group was obviously better than control group, with statistical significance （P〈0.05）. In observation group and control group , the total effective rate was 56.67% and 33.33% respectively, the efficacy of observation group was better than that of control group, with statistical significance （P〈0.05）. Patients of two groups had no serious adverse effect. Conclusion The clinical curative effect of aspirin combined with clopidogrel in the treatment of the acute phase of cerebral infarction is superior to aspirin alone, can delay the progress of the disease.
