摘要
目的探讨宫、腹腔镜联合药物治疗女性不孕症盆腔子宫内膜异位症的治疗效果。方法选择2009年来本院收治的60例不孕症合并子宫内膜异位症的病例,采用宫、腹腔镜检查及手术治疗,随机分为对照组和术后治疗组,对照组术后不用任何药物,术后治疗组在术后月经来潮第1天予以皮内注射戈舍瑞林3.6 mg,每28 d一次,连用3个月。结果对照组显效12例,占40%,有效10例,占33.3%,无效8例,占26.7%,总有效率为73.3%,随访1年中有22例妊娠,占73.3%,治疗组显效15例,占50%,有效12例,占40%,无效3例,占10%,总有效率90%,随访1年中全部患者均成功怀孕,治疗组总有效率及妊娠率明显优于对照组,结果比较差异具有统计学意义(P<0.05)。结论宫、腹腔镜联合戈舍瑞林治疗,操作简便,副作用小,有推广应用的价值。
Objective To investigate treatment effect of hysteroscopy and laparoscopy combined with drug in treatment of infertile women with pelvic endometriosis. Methods Selection of 2009 in our hospital 60 cases of infertility with endometriosis cases, the hysteroscopy and laparoscopic examination and operation treatment, were randomly divided into treatment group and control group after surgery, after surgery in the control group without any drug treatment, postoperative first day of menstrual cramps in the group after operation to intradermal injection of goserelin 3.6mg, once every 28 days, once every 3 months. Results The control group were 12 cases, accounted for 40%, effective in 10 cases, accounting for 33.3%, invalid 8 cases, accounting for 26.7%, the total effective rate was 73.3%, 1 years of follow-up in 22 cases of pregnancy, accounted for 73.3%, accounting for 50% of group therapy in 15 cases, 12 cases, effective, accounting for 40%, invalid 3 cases, accounting for 10%, the total effective rate was 90%, 1 years of follow-up in all patients were successful pregnancy, treatment group total effective rate and pregnancy rate was significantly better than the control group, the results were statistically significant （P〈0.05）. Conclusion Hysteroscopy and laparoscopic goserelin treatment, simple operation, small side effect, has the value of popularization and application.
