摘要
目的 探讨射频热凝联合臭氧消融术治疗极外侧型腰椎间盘突出症的临床疗效.方法 将极外侧型腰椎间盘突出症患者36例,在C形臂引导下穿刺到腰椎间盘突出的髓核内,先注入45μg/ml浓度臭氧10 ml,再用射频仪将温度升高到80 ~90℃行热凝治疗300 s.结果 术后即时和术后1 w时,治疗效果优良率约为70%,随着随访时间的推移,优良率逐步升高,术后24个月时达到94.4%.术后各期VAS分值均明显低于术前,随着时间的增加,VAS分值呈明显下降趋势,术后24个月时,从术前的7.37下降为1.35.结论 射频热凝联合臭氧消融术是治疗极外侧型腰椎间盘突出症的有效方法.
Objective To discuss the clinical curative effects of radiofrequency thermocoagulation and ozone ablation on far lateral lumbar intervertebral disc herniation. Methods Thirty six patients with far lateral lumbar intervertebral disc protrusion received the puncture into the nucleus pulposus of lumbar disc herniation under the guidance of C-arm. Firstly they received the injection with 10 ml ozone with the concentration of 45 μg/ml and then the thermocoagulation for 300 s under the temperature of 80-90 ℃ which was raised by the radio frequency instrument. Results At the end of the operation and 1 w after the operation, the excellent and good rate of curative effect was about 70%. Along with the lasting time of the follow-up, the excellent and good rate increased and reached to 94.4% 24 months after the operation. The VAS scores in each period after the operation were all significantly lower than those before the operation. Along with the increase of time, the VAS scores had the tendency of decrease and decreased from 7.37 before the operation to 1.35 24 months after the operation. Conclusions Radiofrequency thermocoagulation combined with ozone ablation is an effective treatment for far lateral lumbar intervertebral disc herniation.
