摘要
目的探讨妊娠合并血小板减少的病因、临床特点、治疗方法、分娩方式等。方法回顾性分析107例妊娠合并血小板减少患者的临床资料。结果 107例患者中妊娠相关性血小板减少51例(47.66%),特发性血小板减少性紫癜23例(21.50%),巨幼细胞性贫血15例(14.02%),妊娠期高血压疾病导致的血小板减少11例(10.28%),再生障碍性贫血者4例(3.74%),其他疾病导致血小板减少者3例(2.80%)。各类病因的治疗效果无明显差异(P>0.05)。对分娩方式的研究表明,ITP患者的剖宫产率与其他组差异有显著性(P<0.05)。107例血小板减少患者中发生早产的4例(13.08%),正常组30例患者中发生早产的1例(3.33%),差异无统计学意义(P>0.05)。血小板减少组发生产后出血的11例(10.28%),正常组1例(3.33%),差异无统计学意义(P>0.05)。结论妊娠合并血小板减少病因有多种,其中妊娠相关性血小板减少是最常见的原因。血小板计数<30×109/L,同时有出血倾向者主张行剖宫产术。
Objective The aim of this study is to investigate the cause, clinical characteristics, treatment, mode of delivery in pregnant women with thrombocytopenia. Methods To retrospectively analysis the clinical data of 107 patients of pregnant women with thrombocytopenia. Results Among the cases of pregnancy with thrombocytopenia, pregnancy - associated thrombocytopenia are 51 cases （47. 66 %）, idiopathic thrombocytopenic purpura 23 cases （21.50 %）, megaloblastic anemia 15 casesf 14.02 %）, hypertensive disorders in pregnancy 11 cases （10. 28 % ）, aplastic anemia in 4 cases （3. 74% ）, and other causes in 3 patients （2. 80 % ）. The treatment effect of various etiology was no significant difference（ P 〉 0. 05） . Studies about the mode of delivery have shown that the difference between idiopathic thrombocytopenic purpura patients and other group in cesarean rate was significant （ P 〈 0.05）. Premature delivery of 107 cases of thrombocytopenia are 4 casesf 13. 08 % ）, the normal group of 30 patients is 1 case （ 3.33 %）, which in the difference was not statistically significant （ P 〉 0.05）. Postpartum hemorrhage are 11 cases（10. 28 %） in thrombocytopenia group, and for 1 case （3.33 %） in the normal group, which the difference was not statistically significant （ P 〉 0. 05） . Conclusion There are many causes of thrombocytopenia during Pregnancy, which pregnancy - associated thrombocytopenia is the most common cause. When the platelet count〈 30 × 10^9/L while bleeding tendencies we advocated cesarean section.
