利多卡因气管导管外喷洒对苏醒期呛咳反应的影响

摘要 目的术前气管导管外喷洒利多卡因对全身麻醉苏醒期呛咳反应的影响。方法 60例手术时间<1h的腹腔镜下胆囊切除术患者,美国麻醉医师学会分级Ⅰ或Ⅱ级,随机分入对照组和利多卡因组(气管导管外均匀喷洒利多卡因),每组30例。记录两组患者的手术时间(切皮开始至缝皮结束的时间)、苏醒时间(缝皮结束至呼唤姓名后睁眼的时间)、拔管时间(麻醉苏醒至拔除气管导管的时间)、芬太尼用量,以及麻醉苏醒期拔管前后患者的呛咳反应情况。结果两组间手术时间、苏醒时间、拔管时间、芬太尼用量的差异均无统计学意义(P值均>0.05)。对照组有1例患者因自身解剖异常改行开腹胆囊切除术。术毕拔除气管导管前利多卡因组的呛咳反应发生率为23.3%(7/30),显著低于对照组的79.3%(23/29,P<0.05)。拔除气管导管时所有患者均有咳嗽保护发射发生。结论气管导管外喷洒利多卡因可明显减轻短小手术全身麻醉苏醒期的呛咳反应。 Objective To investigate the efficacy of sprinkling lidocaine aerosol on tracheal tube before intubation in attenuating coughing during anesthesia emergence. Methods Sixty patients (American Society of Anesthesiologists I or II ) undergoing laparoscopic cholecystectomy (duration of operation less than 1 h) were randomly divided into control group and lidocaine group (30 cases in each group). 7% lidocaine was sprinkled on the head and cuff of tracheal tube in the lidocaine group, while the tracheal tube in the control group was not treated before anesthesia. Operative time, recovery time, extubation time, fentanyl dose and coughing events in recovery period were recorded. Results There were no significant differences in the operative time, recovery time, extubation time or fentanyl dose between the two groups Call P〉0.05). Cholecystectomy was performed in one patient of control group because of abnormal anatomy. The incidence of coughing during emergence in the lidocaine group (23.3%, 7/30) was significantly lower than that in the control group (79.3%, 23/29, P〈0.05). Cough reflex happened in all patients while extubating. Conclusion Springkling lidocaine on tracheal tube can significantly decrease the incidence of coughing during emergence from general anesthesia for short operation.
