摘要
目的观察黄芪注射液和透明质酸联合O3关节腔内注射治疗OA和单独应用O3治疗的疗效比较。方法将60例膝OA患者分为2组;实验组采用黄芪注射液2.5 mL、透明质酸2.5 mL、浓度为35 mg·L-1的医用臭氧15 mL关节腔注射,对照组采用生理盐水5 mL、浓度为35 mg·L-1的医用臭氧15 mL关节腔注射。治疗后1周抽取关节液,分别检测关节液IL-1β、TNF-α水平;治疗前后对OA患者按照VAS、LKSS等标准比较各组疗效。结果黄芪注射液和透明质酸联合O3关节腔内注射治疗OA和单独应用O3治疗均能很好改善VAS及LKSS评分、降低关节肿胀及活动指数,黄芪注射液和透明质酸联合O3注射较单独O3治疗更明显(P<0.05);同时联合注射与O3单独注射治疗OA比较能明显降低IL-1β、TNF-α的含量(P<0.01)。结论黄芪注射液与透明质酸联合O3关节腔内注射治疗OA较单独注射O3明显降低IL-1β、TNF-α的含量,其通过抗炎减轻OA患者临床症状,改善关节功能。
Objective To observe the efficacy on osteoarthritis（OA）treated with the combined ther-apy of intra-articular injection with Injection with Astragalus Injection,Hyaluronic Acid and O3 as compared with the single application of O3 .Methods Sixty cases of OA were divided into two groups.In the experi-mental group,Astragalus Injection 2.5 mL,Hyaluronic Acid 2.5 mL and medical O3 35 mg＆#183;L-1 ,1 5 mL were injected inside the joint cavity.In the control group,the physical saline 5 mL and medical O3 35 mg＆#183; L-1 ,1 5 mL were injected inside the joint cavity.The synovial fluid was collected after treatment of 1 week to detect the levels of IL-1βand TNF-α.The efficacy was compared according to VAS and LESS before and after treatment in OA patients.Results The combined injection with Astragalus Injection,Hyaluronic Acid and O3 and the single application of O3 all improved VAS and LKSS scores,reduced joint swelling and activi-ty indexes.The efficacy of the combined injection was much more obvious as compared with the single appli-cation of O3 （P〈0.05 ）.The combined injection apparently reduced the levels of IL-1βand TNF-αas compared with the single application of O3 （P〈0.01 ）.Conclusion The combined injection with Astragalus Injection,Hyaluronic Acid and O3 reduce obviously the levels of IL-1βand TNF-αas compared with the single application of O3 and it improves the joint function by anti-inflammation and alleviating the clinical symptoms of OA patients.
