咪达唑仑和右美托咪定抑制七氟醚麻醉苏醒期躁动的对比观察 被引量:11

摘要 目的对比分析咪达唑仑和右关托咪定抑制七氟醚麻醉苏醒期躁动的影响。方法将70例择期行骨科手术的老年患者均分为咪达唑仑组、右美托咪定组,均采用七氟烷吸入麻醉。手术结束前5min,右美托咪定组静脉滴注右美托咪定,咪达唑仑组静脉滴注咪达唑仑,缓慢静脉滴注10min。分别观察两组心率(HR)、呼吸频率(RR)、平均动脉压(MAP)、脉搏血氧饱和度(SpO2)、手术基本情况、疼痛评分、躁动评分及躁动发生率。结果咪达唑仑组的清醒时间为(6.1±1.7)min,右美托咪定组的清醒时间为(6.8±1.6)min(f=1.774,P〈0.05);喉罩拔去时间咪达唑仑组为(6.2±1.5)min,右美托咪定组为(6.9±1.7)min(t=1.827,P〈0.05);术后,咪达唑仑组和右美托咪定组均能有效地改善血流动力学指标,降低HR、RR及MAP水平,但右关托咪定术后的HR、RR、MAP、SpO,均明显优于对照组的(P〈0.05)。结论咪达唑仑和右美托咪定抑制患者的疼痛及躁动效果一样,但咪达唑起效快,而右美托咪定能更有效地控制血流动力学指标,不产生呼吸抑制,安全性更高。
