摘要
目的:比较并评价出经皮冠脉介入治疗和药物治疗不稳定型心绞痛的长期疗效分析。方法:回顾性研究2011年7月至2012年6月入住我院的120例50岁以上不稳定型心绞痛急性发作患者,随机分成对照组和治疗组,每组各60例患者。在住院期间,对照组患者做常规性药物治疗;治疗组患者则在住院期间配合药物治疗基础之上,并进行经皮腔冠状动脉成形术进行治疗。对两组患者不稳定型心绞痛的急性发作和治疗进展做出比较(包括两组患者手术前后CRP、TNFα、cTnI浓度比较)。结果:60例对照组中,患者获得显效为15例,有效32例,无效12例,并有1例发生恶化,临床有效率78.3%;60例治疗组中,手术圆满成功56例,总共植入支架73枚,手术后没有参与狭窄的情况。患者的不稳定型心绞痛在手术后均获得不同程度的减缓,完全恢复为41例,显效34例,有效24例,无效2例,临床有效率达到96.7%。同时,治疗组患者手术前后CRP、TNFα、cTnI浓度发生显著变化,而对照组则无明显差异。结论:在对不稳定型心绞痛患者在药物治疗基础上加用经皮腔冠状动脉成形术进行治疗的方式中,具有显著的成效,值得在临床上大力推广与应用。
Objective：To compare and evaluate the long-term efficacy of percutaneous coronary intervention and drug treatment of unstable angina analysis.Methods：2011 July to 2012 June, retrospective study in our hospital 120 patients aged over 50 with unstable angina in patients with acute episode, were randomly divided into treatment group and control group, each group had 60 patients. In the hospital, patients in the control group routine drug therapy;treatment group patients in the hospital with the basis of drug treatment, and were treated with percutaneous transluminal coronary angioplasty. Comparison of two groups of acute exacerbation in patients with unstable angina pectoris and treatment （including two groups of patients before and after operation of CRP, TNFα, cTnI concentration）. Results：60 cases of the control group, patients were markedly 15 cases, effective 32 cases, invalid in 12 cases, and 1 cases of deterioration, clinical efficiency of 78.3%;60 cases of treatment group, operation success in 56 cases, a total of 73 stents implantation after operation, not involved in narrow circumstances. Unstable angina pectoris were obtained in patients with slow in varying degrees after operation, 41 cases of complete recovery, 34 cases markedly effective, effective 24 cases, invalid in 2 cases, the clinical effective rate reached 96.7%. At the same time, the treatment groups before and after the operation of CRP, TNFα, cTnI concentration changed significantly, while the control group had no significant difference Conclusion：In patients with unstable angina in drug treatment combined with percutaneous transluminal coronary angioplasty in the treatment of the way, has the remarkable effect, worthy of clinical popularization and application.
