Value of combined detection of Cys-C,HbA1c,GA and U-mAlb/Cr in diagnosis of early renal damage in patients with type 2 diabetes mellitus

摘要 目的探讨血清胱抑素C(Cys-C)、糖化血红蛋白(HbA1c)、糖化清蛋白(GA)和尿微量清蛋白/肌酐比值(U-mAlb/Cr)联合检测对2型糖尿病(T2DM)早期肾损伤的价值。方法选取158例T2DM患者(病例组),按UACR分为3组,糖尿病无肾病(A组)、早期糖尿病肾病(B组)、临床糖尿病肾病(C组),另选50例健康者作为对照组,分别测定Cys-C、HbA1c、GA和UmAlb及Cr,评价各项测定结果的关系。结果病例组与对照组比较,Cys-C、HbA1c、GA水平差异有统计学意义(P<0.01);在病例组3组间各项指标比较差异有统计学意义(P<0.01);Cys-C、HbA1c、GA与UACR分别进行直线相关回归,均呈正相关(P<0.01)。结论 Cys-C、HbA1c、GA在T2DM中表达异常,并与病程密切相关,联合检测Cys-C、HbA1c、GA对糖尿病及并发症早期诊断、治疗及预后有重要诊断价值。 Objective To investigate the value of the combined detection of cystatin C (Cys-C), glycosylated hemoglobin (HbAlc), glyeosylated albumin(GA) and urine-microalbumin/ereatinine (U-mAlb/Cr) in the diagnosis of early renal damage in the patients with type 2 diabetes mellitus(T2DM). Methods 158 cases of T2DM were selected as the patients group and divided into 3 groups according to the urine albumin creatinine ratio(UACR) :non-diabetic nephropathy(DN) group (group A), early DN group (group B) and the clinical DN group (group C) ,other 50 cases of healthy people were chosen as the control group. Cys-C, H bAlc, GA, U-mAlb and Cr were detected and the relationship among the determination results was evaluated. Results Cys-C, HbAlc and GA levels had statistically significant differences between the patients groups and the control group (P〈0.01). The statistical differences also existed among the three patients groups. The linear correlation regression analysis showed that Cys-C, HbAlc and GA were positively correlated with UACR (P〈0.01). Conclusion Cys-C, HbAle and GA have abnormal expression in T2DM,and are closely related with the course of the disease, so the combined detection of Cys-C, HbAlc and GA has important diagnostic value in early diagnosis,treatment and prognosis of diabetes and its complications.
