摘要
【目的】观察升清降浊法对维持性血液透析(MHD)患者的微炎症状态的影响。【方法】将45例MHD患者随机分为治疗组23例和对照组22例。对照组采用常规血透治疗,治疗组在常规血透治疗的基础上加用升清降浊中药方(黄芪、茯苓、白术、丹参、何首乌、大黄、炙甘草等)治疗。观察2组治疗前后C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及血浆内毒素水平的变化。【结果】治疗组治疗后微炎症状态各指标CRP、IL-6、TNF-α及血浆内毒素水平较治疗前均明显下降(P<0.05),而对照组的微炎症状态各指标均无明显下降(P>0.05);治疗后2组比较,治疗组的下降作用优于对照组,差异均有统计学意义(P<0.05)。【结论】升清降浊法可以有效改善MHD患者的微炎症状态,提高患者的生活质量。
Objective To observe the influence of therapy of lifting lucid yang and lowering turbid yin on microinflammatory state of maintenance hemodialysis （MHD） patients. Methods Forty-five MHD patients were randomized into treatment group （N =23） and control group （N =22） The two groups were given conventional hemodialysis treatment, and the treatment group was additionally given Chinese herbal prescription with the actions of lifting lucid yang and lowering turbid yin, which is mainly composed of Radix Astragali, Poria, Rhizoma Atractylodis, Radix Salviae Miltiorrhizae, Radix Polygoni Multiflori, Radix et Rhizoma Rhei, and Radix Glycyrrhizae Preparata. Before and after treatment, the changes of serum C-reactive protein C （CRP）, interleukin-6 （IL-6） and tumor necrosis factor alpha （TNF-α） levels as well as the plasma endotoxin level were observed. Results In the treatment group, the microinflammatory indexes of CRP, IL-6, TNF-α and plasma endotoxin were obviously decreased after treatment （P〈O.05 compared with those before treatment） , but the microinflammatory indexes remained steady in the control group （P〉0.05） , and the decrease of the inflammatory indexes in the treatment group was superior to that in the control group （P〈0.05）. Conclusion Therapy of lifting lucid yang and lowering turbid yin is effective on improving microinflammatory state of maintenance hemodialysis patients, and can enhance the quality of life.
