摘要 目的探讨新生儿缺氧缺血性脑病(HIE)早期的MRI表现。方法回顾性分析146例新生儿缺氧缺血性脑病患儿早期的MRI表现。结果头颅MRI提示HIE患儿的早期异常征象主要有:脑水肿94例,88例为局限性脑水肿,表现为额顶叶皮层高信号,灰白质分界不清;基底节、丘脑、内囊后肢异常信号31例,表现为基底节、丘脑背侧斑片状T1信号增高,T2为低信号,内囊后肢正常高信号消失;脑实质出血10例,脑室内出血17例,蛛网膜下隙出血85例;1例重度窒息表现为大面积脑梗死。结论 MRI可以反映新生儿HIE早期脑损伤的多种表现,能反映出新生儿缺氧缺血性脑病的脑损害程度,对新生儿缺氧缺血性脑病诊断有较高的参考价值。 Objective To investigate the relationship between MRI and prognosis in newborns with hypoxic ischemic encephalopathy (HIE). Methods A retrospective analysis was used to detect the early MRI findings of 146 cases of neonatal HIE. Results Cranial MRI prompted early abnormal signs of HIE : (1)94 cases of brain edema,including 88 cases of localized brain edema, character- ized by high signal frontal and parietal cortex and no clear boundary between gray matter;(2)31 cases of abnormal signals of basal gan- glia, thalamus,internal capsule and hind legs, such as T1 signal increased, T2 signal decreased in thalamus patchy, and high signal dis- appeared in the internal capsule hind legs;(3)intracerebral hemorrhage, including 10 cases of parenchymal hemorrhage, 17 cases of in- traventricular hemorrhage, and 85 cases of subarachnoid hemorrhage;(4)one case of a severe asphyxia was characterized by large area of cerebral infarction. Conclusion MRI can reflect the early brain damage of neonatal HIE in a variety of performance, and reflect the degree of brain damage in neonatal hypoxic-ischemic encephalopathy. MRI has a high reference value in the diagnosis of neonatal hy- poxic-ischemic eneephalopathy.
