摘要
目的探讨超声引导下经阴道射频消融术在子宫肌瘤临床治疗中的疗效和安全性。方法对68例子宫肌瘤患者进行超声引导下经阴道射频消融术治疗,术后均进行6个月的随访,测量术后子宫肌瘤体积的变化,对比患者手术前后临床症状的变化情况。结果肌瘤体积方面,治疗后,壁间肌瘤有效率为91.42%(32/35),浆膜下肌瘤有效率为93.33%(14/15),黏膜下肌瘤和宫颈肌瘤有效率均为100%(10/10和8/8),组间差异均无统计学意义(P>0.05);临床症状评估方面,随访3个月时有效率为88.24%(60/68),随访6个月时有效率为98.53%(67/68),组间差异有统计学意义(P<0.05);治疗后,并发症发生率为7.35%(5/68),均经对症治疗痊愈。结论超声介入下经阴道射频刀消融术治疗子宫肌瘤是有效的微创治疗方法,操作简便快捷,临床疗效突出,安全性较高,值得临床推广应用。
Objective To investigate the efficacy and safety of ultrasound-guided transvaginal radiofrequency ablation in the treatment of uterine leiomyoma. Methods Sixty-eight uterine leiomyoma patients were treated with ultrasound-guided transvaginal radiofrequency ablation, and were followed up for 6 months. Results In term of leiomyoma volume, the effective rate was 91.42%（32/35） for intramural leiomyoma, 93.33% （14/15）for leiomyoma under serosa, 100% for both submucous myoma and cervical myoma（10/10 and 8/8）, and no significant difference existed （P＞0.05）.As for clinical symptoms, the effective rate at the 3rd month of follow-up was 88.24%（60/68）, and 98.53%（67/68） at the 6th month, with significant differences （P＜0.05）. Postoperative complication occurred in 5 patients with a rate of 7.35%（5/68）, but all recovered after symptomatic treatment. Conclusion ultrasound-guided transvaginal radiofrequency ablation is an effective microinvasive method in the treatment of uterine leiomyoma, with simple and convenient operation, apparent clinical effectiveness, and high safety, implying its broad application.
