儿童椎管内肿瘤不同术式对脊柱稳定性的影响

摘要 目的:探讨研究儿童椎管内肿瘤手术对脊椎脊柱的稳定性的影响。方法:儿童椎管内恶性肿瘤29例,其中行椎管扩大成形术9例,行椎板复位并钛板固定术20例。术后皆予一定程度的康复训练。术后3年观察远期脊柱稳定性情况。结果:(1)手术椎板节段:A组2.33±0.94,B组2.80±1.21,手术显露与切除:A组5例,B组10例。两组比较差异无统计学意义(P>0.05);(2)JOA运动评分:A组2.22±1.13,B组2.40±1.20,两组比较差异无统计学意义(P>0.05)。两组术后3年均有不同程度的脊柱失稳情况,其中颈段发生率最高,胸段发生率最低。结论:儿童期髓内肿瘤以恶性肿瘤多见,恶性程度低者行椎板复位并钛板固定术,恶性程度高者行椎板切除并扩大成形术。
