摘要
目的介绍痔切除术中应用Hill-Ferguson直肠拉钩结合鼠齿钳暴露手术野的技巧。方法选择本院普外科收治的182例混合痔患者,在行痔切除术时,应用Hill-Ferguson直肠拉钩结合鼠齿钳暴露手术野的方法,严密观察患者术中术野暴露情况及术中、术后并发症发生情况。结果本组患者术中术野暴露清楚,无需第2助手用手牵引臀部,暴露肛门。手术均彻底切除痔核,且术中内括约肌解剖清楚,无1例损伤。术后无肛门出血,远期无肛门失禁、肛门狭窄等并发症。结论在痔切除术中,应用Hill-Ferguson直肠拉钩结合鼠齿钳暴露手术野的技巧,使术野暴露清楚,手术操作方便,患者体位舒适,不仅有效提高了手术安全性,且护理方便。
Objective To introduce the techniques of operative field exposure by applying Hill-Ferguson rectal retractor combined with Allis clamps in hemorrhoidectomy. Methods A total of 182 patients with mixed hemorrhoids were selected in the Department of General Surgery of our hospital. Hill-Ferguson rectal retractor combined with Allis clamps was used to expose the operative field during hemorrhoidectomy. The conditions of intraoperative operative field exposure, intraoper- ative and postoperative complications were strictly observed. Results The operative field was ex- posed clearly in patients during operation, and it was unnecessary for the second assistant to pull the buttock to make the anus expose manually. All haemorrhoids were removed thoroughly by opera- tion, internal anal sphincter was dissected clearly during operation, and no one was injured. Be- sides, there were no complications like archorrhagia, long-term anal incontinence and anal stenosis. Conclusion During hemorrhoidectomy, applying the techniques of Hill-Ferguson rectal retractor combined with Allis clamps makes the operative field expose clearly, operation convenient and pa- tients＇ position comfortable, which not only improves the operative safety effectively, but also nurs- es conveniently.
