摘要
目的观察金匮肾气丸联合美卡素治疗Ⅲ期糖尿病肾病的临床疗效并探讨其可能的作用机制。方法选取60例Ⅲ期DN患者随机分成观察组与对照组,每组各30例,除常规降糖、降压、降脂等治疗,观察组给予金匮肾气丸和美卡素联合治疗,对照组仅给予美卡素治疗。治疗前、后分别检查尿蛋白排泄率,血清肌酐、血清尿素氮含量等肾功能,并在治疗过程中监测空腹血糖和血压等指标。结果两组治疗2个月后患者一般情况无统计学差异(P>0.05),尿蛋白排泄率、血清肌酐含量均有明显下降,差异具有显著性(P<0.01或P<0.001),观察组疗效优于对照组;同组中治疗前后尿蛋白排泄率均明显降低(P<0.001)。结论金匮肾气丸联合美卡素治疗Ⅲ期糖尿病肾病具有降低尿蛋白排泄率从而改善肾功能的作用,值得临床推广。
Objective To observe the clinical effects of combination of Jinkui Shenqi Pill and Mecarbinate in the treatment of stage-lU diabetic nephropathy, to discuss the possible mechanism. Methods Sixty patients with stage-Ill diabetic nephropathy were randomly divided into observation group and control group, each group is 30 cases. In addition to the conventional therapy of hypoglycemic, antihypertensive, lipid,the method of observation group was combination of Jinkui Shenqi Pill and Mecarbinate, the control group was treated with Mecarbinate. Before and after treatment respectively to check urine albumin excretion rate, serum creatinine, serum urea nitrogen and so on renal function, and in the treatment process monitoring indices such as fasting blood glucose and blood pressure. Results Two groups of patients after 2 months of treatment in general, no statistical difference （P 〉 0.05 ）. Urinary albumin excretion rate, serum creatinine levels were significantly lower, with significant difference （P 〈 0. 01 or P 〈 0. 001 ） , the curative effect of observation group is better than the control group; In the same group before and after treatment urinary protein excretion rate were significantly decreased （P 〈 0. 001 ）. Conclusion the clinical effects of combination of Jinkui Shenqi Pill and Mecarbinate in the treatment of stage-Ⅲ diabetic nephropathy can reduce urine protein excretion rate so as to improve the kidney function, it is worthy of clinical promotion.
