摘要
目的 探讨整体护理措施在妊娠期胆汁淤积综合征中的应用效果.方法 分析浙江省建德市第一人民医院妇产科2010年7月～2013年7月收治的妊娠期胆汁淤积综合征80例患者的临床资料,依据护理措施不同将其分为两组,常规护理组30例和整体护理组50例,常规护理组给予妊娠期胆汁淤积综合征孕妇常规护理措施,护理组在常规护理的基础上,通过分析妊娠期胆汁淤积综合征孕妇临床特点给予个性化整体护理.观察两组孕妇年龄分布状态、分娩经历、孕龄、病情分型情况,观察两组孕妇血清总胆红素、谷丙转氨酶、血清胆汁酸情况,观察两组孕妇胎儿窘迫、羊水污染、新生儿窒息、早产、围生儿死亡、新生儿体重情况.结果 两组孕妇年龄分布状态、孕龄、分娩经历、病情分型比较差异均无统计学意义（t=0.30,0.95,x2=0.91,2.20,P＞0.05）,整体护理组孕妇血清总胆红素、谷丙转氨酶、血清胆汁酸、新生儿体重、胎儿窘迫、羊水污染、新生儿窒息、早产、围生儿死亡均优于常规护理组,差异均有统计学意义（t=3.54,6.39,4.20,7.79,x2=16.00,12.50,28.36,23.95,3.36,P＜0.05）.结论 整体护理措施可以明显改善妊娠期胆汁淤积综合症临床症状,提高妊娠结局质量,值得临床推广应用.
Objective To approach application effect of cholestasis syndrome during pregnancy by holistic nursing measures.Methods 80 cases clinical data of cholestasis syndrome during pregnancy from July 2010 to July 2013 in Department of Obstetrics and Gynecology of the First People＇s Hospital of Jiande City in Zhejiang Province,were divided into conventional nursing group of 30 cases and holistic nursing group of 50 cases.Routine nursing group were given routine nursing for cholestasis syndrome during pregnancy pregnant women,nursing group were given personalized nursing as a whole on the basis of conventional nursing,through analyzing the clinical characteristics of pregnant women with the pregnancy cholestasis syndrome.The age distribution,delivery,gestational weeks,disease type of two groups gravida were detected,the serum total bilirubin,alanine transaminase,serum bile acid of two groups gravida were detected,the fetal distress,amniotic fluid contamination,neonatal asphyxia,preterm birth,perinatal mortality,neonatal weight of two groups gravida were detected.Results The age distribution,gestational weeks,delivery,disease type of gravida in two groups were not difference （t =0.30,0.95,x2=0.91,2.20,P ＞ 0.05）,the serum total bilirubin,alanine transaminase,serum bile acid,neonatal weight,fetal distress,amniotic fluid contamination,neonatal asphyxia,preterm birth,perinatal mortality of holistic nursing group gravida were better than conventional nursing group,the differences were statistical significant （t =3.54,6.39,4.20,7.79,x2=16.00,12.50,28.36,23.95,3.36,P ＜ 0.05）.Conclusion The stage of cholestasis syndrome clinical symptoms of pregnancy is obviously improve by holistic nursing measures,increased pregnancy outcome quality,which is to be clinical used.
