摘要
目的 探讨硬腭黏膜移植联合周围皮瓣转移修复眼睑全层缺损的临床疗效.方法 对2011年3月～2013年3月,于丽水市人民医院眼科诊治的12例眼睑恶性肿瘤（基底细胞癌6例,睑板腺癌2例,鳞状上皮细胞癌2例,恶性黑色素瘤2例）行冰冻切片监控肿瘤边缘切除后全层眼睑缺损的患者,术中用自体游离硬腭黏膜移植片替代睑板和结膜,再造后层眼睑;转移周围皮瓣覆盖于硬腭黏膜之上,代替缺损的眼睑皮肤进行眼睑重建.术后对眼睑形态、眼睑功能进行观察.结果 术后随访6～24个月,所有患者硬腭黏膜植片及转移皮瓣全部成活,眼睑形态及功能均基本恢复,眼睑活动自如,闭合良好,无挛缩,无眼睑内外翻畸形,无一例复发.结论 采用硬腭黏膜移植联合周围皮瓣转移,可以一次性修复恶性肿瘤切除术后全层缺损的皮肤、睑板、结膜,效果肯定,值得推广.
Objective To investigate the outcome of combining grafting of hard palate mucosa and periocular skin flap in the reconstruction of full-thickness eyelid defects.Methods From March 2011 to March 2013,in People＇s Hospital of Lishui City,12 cases of eyelid malignant tumor （6 cases of basal cell carcinoma,2 cases of meibomian gland carcinoma,2 cases of squamous cell carcinoma,2 cases of malignant melanoma） were resected the posterior layers of the eyelid under the monitoring of the tumor edge by frozen section,autologous free hard palate mucosa was grafted to replace the tarsal and the conjunctiva to reconstruct the posterior layers of the eyelid; periocular skin flap was transferred to cover the hard palate mucosa instead of the eyelid skin defects.The eyelid morphology and the eyelid function were observed after surgery.Results Patients were followed-up for 6 to 24 months postoperatively,the hard palate mucosal grafts and the flaps existed well in all patients,the eyelid morphology and function basically recovered,eyelids had freedom activity,closed well,no contracture,no inverted or everted eyelids deformity,no relapse.Conclusion Combining grafting of hard palate mucosa and periocular skin flap can repair full-thickness eyelids defects （including the skin,the tarsal and the conjunctiva） after removal of malignant tumors,which as a one-stage operation with definite and good outcome is worth promoting.
