摘要
目的:研究丁苯酞软胶囊与氯吡格雷片联合治疗急性脑梗死的疗效与安全性及经颅多普勒(TCD)脉动指数PI的变化。方法:随机选取颈内动脉系统急性脑梗死患者120例,分为治疗组和对照组,各60例。治疗组口服氯吡格雷片75 mg,每日1次,丁苯酞软胶囊0.2 g,每日3次,对照组口服氯吡格雷片75 mg,每日1次,连续14 d。观察两组治疗前后神经功能缺损评分、生活能力等级评分及不良反应,并通过TCD观察颅内血管的平均血流速度(Vm)及脉动指数的变化。结果:丁苯酞软胶囊联合氯吡格雷片治疗组神经功能缺损评分和生活能力等级较治疗前明显改善(P<0.01),优于对照组(P<0.01);与治疗前相比,Vm明显增快,PI明显降低(P<0.05)。结论:丁苯酞软胶囊联合氯吡格雷片是治疗急性脑梗死安全有效的方法。
Objective：To study butylphthalide soft capsules combined with clopidogrel in the treatment of acute cerebral infarction curative effect and safety of transcranial doppler （TCD） and the changes of pulse index pulsatility index （PI）. Meth- ods：Random selection of internal carotid artery system in 120 patients with acute cerebral infarction. Divided into treatment group and control group with 60 cases in each group,the treatment group oral clopidogrel tablets 75 mg, 1 times one day,bu- tylphthalide soft capsules,0.2 g, 3 times one day; 60 cases of the control group, oral administration of clopidogrel, 75 mg, 1 times one day, for 14 days. Two groups were observed before and after treatment, the neurological deficit scores, life ability score and adverse reactions. And the average flow velocity（Vm） of intracranial vessels and PI changes were observed by TCD. Results：Butylphthalide soft capsules combined with clopidogrel treatment group, neurological deficit score and life ability level significantly improved compared with before treatment （ P 〈 0.01 ）. Better than the control group （ P 〈 0.01 ）. Compared with before treatment, Vm increased obviously, the PI decreased significantly （ P 〈 0.05 ）. Conclusion ： Butylphthalide soft capsules combined with clopidogrel tablet is an effective and safe method in the treatment of acute cerebral infarction.
