摘要
目的:应用消化道粘膜血流图像处理技术定量研究胃角溃疡及其周边粘膜血流,并对溃疡愈合时间进行预测。方法:由计算机自动计算电子内镜图像上每一像素的血红蛋白浓度指数(HI=32In Vr/Vg,Vr:红图像灰度值,Vg:绿图像灰度值);根据溃疡及其周边充血粘膜的HI计算溃疡面积和充血粘膜面积(像素数),溃疡面积与充血粘膜面积比值的对数值称为溃疡愈合指数。予13例胃角溃疡患者以兰索拉唑治疗,对同一患者,利用治疗前、后两次内镜检查测得的溃疡愈合指数和检查时间预测溃疡愈合时间,并在预测的溃疡愈合时间范围内,观察其实际愈合情况。结果:溃疡愈合过程中,充血粘膜面积逐渐增大,并于愈合1期达到峰值,至瘢痕期明显下降。溃疡愈合指数(y)与溃疡发生的时间过程(x),指从溃疡发生至进入瘢痕期的时间)呈直线相关关系(y=-0.15x+0.80, r=0.89,P<0.001),溃疡愈合指数减少一半的平均时间为4.00天±1.92天;在预测的溃疡愈合时间范围内,所有溃疡均愈合。结论:消化道粘膜血流图像处理技术能定量、客观地预测溃疡愈合时间,可用于临床抗溃疡药物的疗效判定。
Backgroud/Aims: To study the blood flow changes of gastric angular ulcer and predict the time of ulcer-healing using mucosal blood flow image processing system. Methods: The hemoglobin-index (HI) of each pixel was calculated using an algorithm (HI=32 In Vr/Vg, Vr:intensity of red image, Vg: intensity of green image). According to the HI, ulcer area, hyperemic area and ulcer-healing index [In (ulcer area)/(hyperemic area)] could be calculated. 13 cases of gastric angular ulcer were treated by lansoprazole, electronic endoscopic pictures of ulcer were taken and analyzed by image processing system to observe the relationship between the ulcer-healing index and the process of ulcer-healing. Results: During ulcer-healing, the hyperemic area increased and reached a peak at the healing stage 1,then,it declined at the scarring stage. The ulcer-healing index was well correlated with the staging of the ulcer. The regression line between the ulcer-healing index (y) and the process of ulcer-healing (x) was y= -0.15x+0.80,r=0.89, P<0.001. The half-life period of ulcer-healing index was 4.00 days±1.92 days. The day entering scarring stage could be predicted according to the first and second ulcer-healing index. The results showed all the ulcers entered scarring stage in the predicted time. Conclusions: Using the electronic endoscopy and image analyzing system, the time of ulcer-healing can be estimated quantitatively and objectively, and the effect of drugs on ulcer-healing can be dynamically estimated.
