摘要
目的:探讨血浆脑钠肽(BNP)、C反应蛋白(CRP)、肌钙蛋白I(cTNI)水平与慢性阻塞性肺疾病急性加重期(AECOPD)患者危险分层及预后的关系。方法:选择AECOPD患者236例及健康对照组30例,按照2007ATS共识对患者进行评估分级,分为轻度、中度和重度三级,分别测定其血浆BNP、CRP和cTNI浓度。同时统计住院病死率、6个月内病死率、6个月内再入院率,进行统计学分析。结果:①AECOPD组BNP(391.73±25.8)ng/L、CRP(19.74±1.63)mg/L、cTNI(0.19±0.046)μg/L显著高于对照组BNP(69.38±8.5)ng/L、CRP(7.15±0.86)mg/L、cTNI(0.10±0.031)μg/L,均P<0.05。②随着AECOPD患者病情加重(分级上升),血浆BNP、CRP和cTNI水平升高,住院病死率、6个月病死率和6个月内再入院率升高,3组之间的差异有统计学意义(P<0.05)。结论:血浆BNP、CRP和cTNI测定有助于AECOPD的危险分层和预后评估。
Objective: To investigate the relation between plasma brain natriuretic peptide (BNP), C-reactive protein (CRP) ,Cardiac troponin I (cTNI) and risk stratification or prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Method:We choosed 236 patients with AECOPD 236 cases and 30 healthy controls 30 cases were recruited. ,and estimated these These patients according to the 2007 ATS con- sensus, thenwere divided them into 3 grades according to the 2007 ATS consensus:mild, moderate and severe level. The plasma BNP,CRP and cTNI concentration were determined detected in all these patients and healthy controls cases. At the same time, The statistical analysis of inpatient mortality,mortality and rehospitalization rates within 6 months after discharge from hospital were down analized. Result:In AECOPD group,the Plasma concentration of BNP (391.73±25.8 ng/L) ,CRP (19.74±1.63 mg/L) ,cTNI (0. 19±0. 046 ILg/L) is significantly higher than that in control group (BNP P=0. 001,CRP P=0. 026,cTNI P=0. 008). In AECOPD group, the levels of BNP, CRP and cTNI showed a positivity relationshipwere positively related with patient classification and , inpatient mortality,mortality and rehospitalization rates with 6 months after discharge from hospital. Conclusion:Determina- tion Detection of plasma concentration of BNP,CRP and cTNI could help us to do evaluate risk stratification and prognosis evaluation of patients with AECOPD.
