摘要 目的:探讨失效模式和效应分析(FMEA)在药物安全管理过程中的应用效果,消除药物从医生开具处方到使用过程的用药缺陷。方法:应用FMEA对医院药物给药过程中可能存在的安全隐患进行分析,选择其中失效模式优先风险指数(RPN)值最高的7个失效模式,分析失效原因和失效可能造成的后果,建立分析模型,制订改善措施并实施。结果:经过1年的工作实践,各项失效模式RPN评分均降到了30分以下。结论:通过FMEA的应用,将药物安全管理过程中失效模式的重要度加以量化,从而指明了药物安全管理改善的优先顺序,使用药安全性得到提高。 OBJECTIVE ty management process, To investigate the effect of application of failure mode and effect analysis (FMEA) in medicine safe to eliminate the defects from issuing orders to the using medicine. METHODS By using FMEA, the possible security risks occurred in hospital medicine use was analyzed; according to the risk priority number (RPN), seven high failure modes were selected, analyzing the failure causes and consequences, to establish the analysis model, to develop and im plement the improvement measures. RESULTS After one year of practice, the failure mode of RPN were reduced to below 30 points. CONCLUSION Through the application of FMEA, the degree of importance of the failure mode in the medicine safety management process was quantified, thereby indicated the priority in improvement of the management of drug safety, and im proved the medication safety.
