摘要
目的:探讨锁定加压钢板(LCP)结合人工骨植入治疗桡骨远端骨折的效果。方法:选取2009年1月至2011年10月的桡骨远端骨折(AO分类C3型)患者24例,所有患者均掌侧入路LCP固定,均予骨缺损处人工骨植入。结果:术后随访12~26个月,平均20.9个月,所有骨折均骨性愈合,愈合时间10~13周,平均11.4周。按改良Gartland-Werley功能评估系统进行疗效评定,优良率为87%。随访结果显示,3例患者桡骨远端高度丢失,有严重的腕关节屈伸功能障碍。结论:LCP结合人工骨植入治疗桡骨远端骨折时,能够坚强固定,允许早期功能锻炼,尤其适用于骨折压缩粉碎性骨质疏松患者。
Objective: To explore the application and outcome of locking compression plate combined with human bone grafting for treatment of distal radius fracture. Methods: From January 2009 to October 2011, twenty- four patients with consecutive distal radius fractures were preformed with open reduction and locking compression plate fixation combined with human bone grafted. 24 patients were treated by the locking plate fixations through simple volar approach and bone grafts. Results: The patients were followed up for average 20. 9 months (range, 12-26 months) ,and all patients got bone union in a range of 10 - 13 weeks, averaging of 11.4 weeks. According to Gartland-Werley functional assessment system, the excellent and good rate was 87%. Follow-up in 3 cases of patients with distal radial height loss, severe carpal joint flexion dysfunction appeared. Conclusion: The locking compression plate combined with human bone graft can provide firm fixation and allow early functional exercise and hence is suitable for unstable distal radius fracture especially the osteoporosis patients with comminuted compression fracture.
