摘要
目的 总结经尿道前列腺电汽化术 (TVP)治疗 70岁以上老年前列腺增生症 (BPH)患者的经验 ,并进行评价。 方法 对 96例 70～ 95岁老年BPH患者 ,经术前充分准备后行TVP治疗 ,并于术后 3个月、6个月进行随访。 结果 TVP平均手术时间 48min ,平均出血量 88ml。术后无需膀胱持续冲洗 ,留置导尿平均 4 5d ,拔除导尿管后均自行排尿。术后平均住院天数 6 5d。术后 3个月及 6个月随访 ,国际前列腺症状评分 (I PSS)降至 9 8分和 6 5分 ,最大尿流率上升为 13 6和 15 8ml/s,残余尿降至 2 8 5和 12 0ml(P <0 0 1)。 结论 TVP为一种安全且疗效确切的手术技术 ,适用于老年BPH患者 ,尤其高龄及伴有重要器官损害而不宜行开放手术的患者 ,经TVP治疗能有效解除尿路梗阻 ,提高生活质量。
Objective To summarize and evaluate the transurethral electrovaporization of prostate (TVP), a minimally invasive method for elderly patients with benign prostatic hyperplasia(BPH). Methods After full preparation,TVP was performed on 96 cases of 70 95 year old BPH patients, and all patients were followed up for 3 6 months after operation. Results On the average,the operation time was 48 minutes, blood loss 88 ml, catheterization time was 4 5 days. The patients had satisfactory voiding after removal of the catheter and the average post operative hospital stay was 6 5 days. By 3 6 months of follow up, I PSS score dropped to 9 8 and 6 5; peak urine flow increased to 13 6 15 8 ml/s, residual urine dropped to 28 5 12 0 ml( P <0 01). Conclusions TVP is a safe and reliable method for BPH patients, especially those over 70 years with dysfunction of vital organs and being unable to undergo open operation. TVP can relieve the bladder outlet obstruction and improve the life quality.
