摘要
目的探讨透明质酸对体外受精周期中胚胎种植情况及围产期状况的影响。方法对626例接受体外受精/卵胞浆内单精子注射(IVF/ICSI)治疗并进行新鲜胚胎移植的不育患者随机分组,使用富含透明质酸的EmbryoGlue作为移植液的296名患者为实验组(A组),使用不含透明质酸的G-2作为胚胎移植液的330例患者作为对照组(B组),比较两组临床妊娠率、胚胎种植率、多胎妊娠率、流产率及异位妊娠率等胚胎种植结局,比较早产率、出生体重、男女比例、剖宫产率、新生儿出生缺陷及新生儿死亡率等围产期状况。结果 (1)两组患者的一般临床资料相似,无统计学差异。(2)实验组患者的种植率较对照组明显升高(35.62%vs.25.95%,P<0.05);临床妊娠率较对照组明显升高(55.07%vs.41.82%,P<0.05);多胎妊娠率较对照组明显升高(22.30%vs.13.64%,P<0.05);异位妊娠率较对照组明显降低(2.45%vs.7.25%,P<0.05);(3)两组围产期状况如早产率、男女比例、胎龄、出生体重、新生儿出生缺陷等无统计学差异。结论富含透明质酸的EmbryoGlue移植液可以促进IVF-ET中胚胎种植,并可降低异位妊娠的发生,不增加新生儿出生缺陷,死亡率、早产率等。
Objective： To discuss the efficacy of using an embryo transfer medium enriched with Hyaluronic acid （HA） to improve implantation. Methods. A total of 626 infertile patients who underwent IVF or ICSI were analyzed retrospectively. Among them,296 patients were used commercial HA enriched embryo transfer medium in study group,and 330 patients were used routine embryo transfer medium without HA in control group. Results. （1）The clinical characteristics of patients are not significantly different between study group and control group. （2） The implantation rate （35.62% vs. 25.95%）, clinical pregnancy rate （55.07%vs. 41.82%）,and multiple pregnancy rate（22.30% vs. 13.64 %）in study group were significantly increased compared with the control group （P 〈 0.05）. The ectopic pregnancy rate was significantly decreased compared with the control group（2.45% vs. 7. 25%）（P〈0.05）. （3）No statistically significant differences were found in average gestational age, birth weight,male-to-female（M/F）ratio, birth defect and neonatal death rate between the study and control groups（P〉0.05）. Conclusions： Usage of HA enriched embryo transfer medium has the beneficial effect to improve the implantation of embryo in IVF-ET and reduce the occurrence of ectopic pregnancy, and dose not increase the birth defect rate, neonatal death and premature birth.
