摘要
目的 探讨扩大足底内侧皮瓣修复足跟恶性黑色素瘤切除后创面的可行性.方法 对6例患者行足跟恶性黑色素瘤扩大切除术,切除后缺损大小为7.0cm＆#215;7.0cm～9.0cm＆#215;9.0cm,采用带蒂扩大足底内侧皮瓣对缺损进行修复.供区游离植皮.结果 所有皮瓣全部成活.随访11～36个月,皮瓣色泽、质地与外形良好,皮瓣的两点辨别觉与对侧足内侧区无明显差异,患肢活动自如.结论 扩大足底内侧皮瓣可获取足够的组织量覆盖足跟及其较大面积的缺损,术后感觉正常,是修复足跟恶性黑色素瘤术后创面的较理想皮瓣.
Objective To investigate the feasibility of the expanded medial plantar flaps in repairing de- fect caused by resection of malignant melanoma in the heel. Methods Six patients with defects who had malig- nant melanoma in the heel region were treated by radical excision and then the defects ranged from 7.0 cm x 7.0 cm to 9.0 cm x 9.0 cm were repaired with expended medial plantar flaps with pedicle. The defects in the do- nor area were repaired with free skin transplantation. Results All the flaps survived postoperatively. After 11 to 36 months following up, all were satisfied with the color, texture and appearance of the flaps and affected limb with free activity. The flaps of two point discrimination were not different with opposite medial region. Conclu- sion The sufficient soft tissue can be gained from the expanded medial plantar flaps to cover the heel region and its large tissue defect with good sensation after operation and prevention of ulcer development. It is an ideal flap to repair soft tissue defect caused by resection of malignant melanoma in the heel.
