摘要
目的：检测湿疹患儿常见的过敏原，寻找儿童湿疹可能的过敏促发因素，为其治疗及预防提供依据。方法：采用Mediwiss公司AllergyScreen Analytic GmbH过敏原检测系统，对872例湿疹患儿进行血清特异性IgE（specific IgE，sIgE）和总IgE检测，并在不同年龄组间进行比较。结果：872例湿疹患儿sIgE阳性690例（79．13％），总IgE阳性709例（81．30％）。其中吸入性过敏原最常见的依次为户尘螨（34．60％）、点青霉（22．71％）、屋尘（16．28％）和狗毛皮屑（10．89％）；食物性过敏原中，最常见的依次为牛奶（36．12％）、羊肉（22．94％）、鸡蛋白（20．07％）和牛肉（13．88％）。比较〈1岁组，1—3岁组和〉3岁组，户尘螨、屋尘、狗毛皮屑、点青霉、鸡蛋白、牛奶、羊肉和蟹的sIgE阳性率的差异有统计学意义（P〈0．05）。结论：户尘螨、点青霉和屋尘是湿疹患儿最主要的吸入性过敏原，牛奶、羊肉、鸡蛋白是湿疹患儿最常见的食物性过敏原。随年龄增大，吸入性过敏原逐步取代食物成为主要过敏原。血清sIgE检测为湿疹患儿提示了相关过敏原，可以针对性地回避相关过敏原。
Objective： To identify the possible allergy triggers via detecting the common allergens in children with eczema, providing scientific basis for the treatment and prevention of eczema. Methods： Serum allergen-specific IgE（sIgE） and total IgE（tIgE） were detected by Allergy Screen detection system in 872 children with eczema and were compared among children in different age groups. Results： Six hundreds and ninety of 872（79.13%）children with eczema were positive for sIgE, and 709 of 872（81.30%）were positive for tlgE. The common aeroallergens were dermatophagoides farinae（DF） （34.60%）, penicil- lium notatum （22.71%）, house dust （16.28%） and dog dander （10.89%）; The common food allergens were milk （36.12%）, mutton （22.94%）, egg white（20.07%）and beef（13.88%）. The positive rates for sIgE of DF, house dust, dog dander, penicilli- um notatum, egg white, milk, mutton and crab were significant different among children aged 〈1, 1-3 and over 3 years（P〈 0.05）. Conclusions： The major aeroallergens are DF, penicillium notatum and house dust in children with eczema, and the common food allergens are milk, mutton and egg white. Aeroallergens are becoming the major allergens as children getting older. Serum slgE tests can identify relevant allergens for patients with eczema to avoid them.
