摘要
目的探讨碎裂QRS(fQRS)波在心肌梗死患者预后评价中的应用价值。方法将200例心肌梗死患者根据心电图检测分为无fQRS组和fQRS组各100例,对2组患者进行20个月的随访,比较2组患者心源性死亡与心血管临床事件的发生率。结果 200例心肌梗死患者中,发生心血管事件85例,占42.5%。fQRS组患者心血管事件的发生率明显高于无fQRS组,差异有统计学意义(P<0.05)。fQRS组发生心源性猝死、非猝死性心性死亡、再发心肌梗死、恶性心律失常及心力衰竭的发生率高于无fQRS组,差异均有统计学意义(P<0.05)。2组心源性猝死发生的相对危险度为3.245,非猝死性心性死亡发生的相对危险度为2.761,再发心肌梗死相对危险度为2.664,恶性心律失常相对危险度为2.897,心力衰竭相对危险度为3.209。fQRS波与患者的年龄、高血压、病变支数、高血脂病史以及糖尿病等均无明显的相关性(P>0.05)。结论心肌梗死预后和fQRS有密切的关系,是对心源性死亡以及心血管事件进行独立预测的一个新指标。
Objective To investigate the application value of fragmented QRS (fQRS)wave in patients with myocardi- al infarction prognosis. Methods According to the electrocardiogram detection,200 patients with myocardial infarction were divided into non fQRS group and fQRS group, each of 100 cases. Followed up for 20 months. Compared the incidence of cardiac death and cardiovascular clinical events of two groups. Results Among 200 cases of patients, cardiovascular events occurred in 85 cases(42.5% ). The cardiovascular events incidence of fQRS group was higher than non fQRS group,the difference was statistically significant( P 〈 0.05 ). The incidence of sudden cardiac death, n.on-sudden death of cardiac death, recurrent myo- cardial infarction, malignant arrhythmias and heart failure of fQRS group were higher than non fQRS group, the differences were statistically significant ( P 〈 0.05 ). The relative risk of sudden cardiac death occurred two groups was 3. 245, the relative risk of non-sudden death cardiac death occurred was 2. 761, the relative risk of recurrent myocardial infarction was 2. 664, the relative risk of malignant arrhythmias was 2.897, the relative risk of heart failure was 3. 209. The fQRS wave with age, hypertension, number of diseased vessels, history of high cholesterol and diabetes had no significant correlation ( P 〉 0.05 ). Conclusion The prognosis of myocardial infarction and fQRS are closely related, is a new independent predictors of cardiac death and cardi- ovascular events.
