摘要
目的探讨无晶状体硅油眼保留晶状体前囊对眼压和角膜内皮的影响。方法临床病例对比研究。对2011年6月至2012年7月在安阳市眼科医院就诊的40例（40只眼）V—VI期糖尿病视网膜病变病人行玻璃体切除、晶状体切除联合硅油填充术，其中26例保留晶状体前囊为保留前襄组；14例晶状体全切为晶状体全切组。两组分别测量术前、术后7d、1个月、3个月眼压值和角膜内皮细胞计数，采用单因素方差分析对数据进行统计。结果保留前囊组术后7d眼压大于术前且差异有统计学意义（P=0．004，P〈0．05），术后1个月、3个月眼压与术前比较差异无统计学意义（P=0．943、0．996，P〉0．05）；该组术后7d、1个月时角膜内皮细胞均数与术前比较差异无统计学意义（P=0．996、0．953，P〉0．05），术后3个月时角膜内皮细胞均数〈术前且差异有统计学意义（P=0．001，P〈0．05）。晶状体全切组术后7d、1个月、3个月眼压均高于术前差异有统计学意义（P=0．012、0．004、0．044，P〈0．05），该组术后7天时角膜内皮细胞均数与术前相比差异无统计学意义（P=0．586，P〉0．05），术后1个月、3个月时角膜内皮细胞均数〉术前差异有统计学意义（P=0．033、0．002，P〈0．05）。两组之间比较，术前两组眼压和角膜内皮细胞计数比较差异均无统计学意义（P=0．887、0．859，P〈0．05）。术后7d两组眼压比较差异无统计学意义（P=0．171，P〉0．05），术后1个月、3个月两组眼压比较差异有统计学意义（P=0．000、0．004，P〈0．05）；术后7d两组角膜内皮细胞计数差异无统计学意义（P=0．308），术后1个月、3个月两组角膜上皮细胞计数比较差异有统计学意义（P=0．019、0．029，P〈0．05）。结论硅油填充术中保留晶状体前囊与晶状体全部切除相比较，前者眼内压较稳定，并在短期内（1个月）较后者能够明显保护角膜内皮细胞。
Objective To discuss the change of intra-ocular pressure （IOP） and corneal endothelial due to saving crystal former capsule in the eye filling silicone oil. Methods Using the method of vitrectomy, lens resection and filling silicone oil to deal with 40 eyes of diabetic retinopathy, there were 26 eyes saved lens anterior capsule and 14 eyes removed all of lens. Intra-ocular pressure and corneal endothelium were measured. These results were analyzed by variance analysis. Results There were no significant difference in saving lens anterior capsule group （SAC） between preoperative and postoperative of IOP at 1 month and 3 months （P =0.943, 0.996）, but significant difference at 7th day （P =0.004）. There were no significant difference in SAC between preoperative and postoperative of corneal endothelial cells at 7th day and 1 month （P =0.996, 0.953）, but significant difference at 3 months （P =0.001）. There were significant difference in removing all lens group （RAL） between preoperative and postoperative of IOP at 7th day, 1 month and 3 months （P =0.012,0.004, 0.044）. There were no significant difference in RAL between preoperative and postoperative of corneal endothelial cells at 7th day （P =0.586）, but significant difference 1 month and 3 months （P =0.033, 0.002）. There were significant difference between the two groups in postoperative lOP at 1 month and 3 months （P =0.000, 0.004）, and significant difference in postoperative corneal endothelial cells at 1 month and 3 months （P =0.019, 0.029）. Conclusions SAC is more effective than RAL in lOP smooth and corneal endothelial cells protection.
