摘要
目的 :探讨无功能胰岛细胞瘤的诊断和治疗。方法 :回顾分析 19例患者的病史资料 ,其中施行肿瘤摘除 5例 ,胰体尾切除 +脾切除 5例 ,胰十二指肠切除 2例 ,胰体尾切除 +Roux- en- Y吻合术 1例 ,全胰切除 1例 ,胆囊空肠吻合术 3例 ,活检 2例。结果 :肿瘤位于胰头 6例 ,胰体 2例 ,胰尾 9例 ,累及全胰 2例 ,多发 1例 ;病理证实恶性 8例 ,良性 11例。除 2例术后分别死于肝功能衰竭和胰瘘、多器官衰竭外均获满意疗效。结论 :手术切除肿瘤均可取得良好疗效。
Objective:To study the diagnosis and treatment of nonfunctioning islet cell tumor retrospectively. Methods:The nineteen cases with nonfunctioning islet cell tumor from 1968 to 1999 year in the First Affiliated Hospital of Nanjing Medical University were studied, with a mean of 32.3 years (range, 7~53). CT, B US, ERCP, MRI and the barium meal of upper Gastrointestinal were performed in 8,14,6,1,10 cases seperatively; and 14 patients were underwent a resection operation. There were pancreaticoduodenectomy 2 cases, distal pancreatectomy plus splenectomy 5 cases, tumor excision 5 cases, tumor excision plus ROUX EN Y anatomsis and whole pancreatectomy 1 case respectively. Results:There were 8 cases confirmed to be malignant; The mass in the head of pancreas was 6 cases, in the body 2 cases, in the tail 9 cases and in whole pancreas 2 cases. All 14 cases had a good prognosis except two cases died from the complications after operations. Conclusion:All the patients with nonfunctioning islet cell tumor ought to resect mass if can.[
